Job summary
An exciting opportunity has arisen for Central Manchester Networks ltd to recruit a PCN Dietitian to base within West Central Manchester PCN. WCM PCN is a group of GP practices working across Central Manchester. Primary care networks think about the wider health of their population, taking a proactive approach to managing population health. By better understanding their wider communities, Primary Care Networks are able to focus on preventative care by tackling health inequalities and identifying those who would benefit from targeted, proactive support.
Main duties of the job
The purpose of this flexible and exciting role is to deliver a bespoke community-based dietetic programme, which best meets the needs of WCM PCN service-users. This role is one of an enthusiastic, creative and innovative practitioner who works autonomously, but also involves liaison with other professionals, both in primary care and relevant community services.
Deliver a programme of dietetic support which meets the unique and diverse needs of service-users from across the Primary Care Network. The current programme running Real Food Real Health encompasses a person centred, strength based approach to helping the community to understand about their diet and how this impacts their health. It is currently being delivered by a health coach and a dietitian from a neighbouring PCN and producing excellent results.
Deliver services in a variety of settings, including but not limited to: community clinics, GP surgeries, patients own-homes, community venues, and group settings.
Promote the role of the dietitian within the GP surgeries and wider network by attending meetings and events to network with other community workers.

This exciting opportunity is to work directly within one network within the locality to support patients from varying background and patient demographics.
