Job summary
An exciting opportunity to work within an evolving innovative and forward-facing clinical model for
general practice. Roles for dynamic GPs who want to be a part of this movement are available at
PCNs, with roles across GP practice sites.The role is open to GPs who are newly
qualified with less than two years of post-qualification experience.
Main duties of the job
Place of work: Islington Central 1, Central 2 or South PCNs
Reports to: Operational Lead of PCN
Accountable to: Clinical Director of PCN
At its core, you will work as a member of the in-house clinical teams and provide a critical role of
delivering appointments and care to the registered list of patients. Although you may not hold a
named doctor list, you will manage the patients with a list-holding mentality, offering continuity
and case management where needed and appropriate. You will play a key role within this, helping
to ensure patients get the right care they need from the right person at the right time. You will
manage admin such as blood results and be part of a team to collectively manage shared admin
alongside other GPs and other staff members. Responsibilities will reflect requirements of the NHS
GP Contracts, and as such may be subject to change as services adapt. This post offers the
opportunity for role and career development, especially with regards to training and supervision of
the wider MDT as the team and service evolves. The role may also involve complex care depending
on your interests and skills.
About us
The PCNs serves a population of between 40,000 and 76,000 patients in the Central and South
Localities in Islington and is support by Islington GP Federation. The PCNs group together
independent GP practices, brought together by the Network Contract Direct Enhanced Service
(DES). PCNs focus on improving patient care by working with partner providers to manage the
needs of their local patient population.
Islington GP Federation (IGPF) is a growing organisation representing 31 practices; we have established ourselves as a leader in new ways of working, including running Islingtons extended access primary care services (I:HUB) as well as supporting the Islington Primary Care Networks (PCNs).
Our current range of services include the Extended Access Service, I:HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms.
IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub and has been working for over two years to create training and development programmes that meet the needs of staff working in primary and community care settings.
IGPF runs four Islington GP practices, one on a GMS contract, two on APMS contracts and one caretaking contracts

- Refer to other Community services and secondary care as appropriate
- Complete comprehensive notes, request the appropriate investigations and develop a
management plan with the patient
- Support the administrative team with clinical and non-clinical queries
- Participate in regular clinical meetings, clinical governance, audits, quality improvement,
training and education.
Your work will particularly support continuity of care, safety and quality for complex or urgent cases:
- You will take responsibility for managing complexity and facilitating continuity of care where
appropriate
- Informing other GPs about complex cases known to them
- Display your clinical reasoning clearly in the records
Clinics
Once a patient has been triaged and assessed as appropriate, they will be scheduled for an initial
consultation, either in person at the practice or via telephone/virtual consultation.
Clinicians document their consultations within EMIS, the primary care electronic record system. This
ensures seamless access to the patients medical history and timely communication with other
healthcare providers, such as GPs or specialists, ensuring follow-up actions are visible to all involved
in the patient's care. Templates are available within EMIS to streamline documentation, and each
clinician will receive an induction to familiarise themselves with the system to optimise its use.
PCN GPs are expected to review incoming clinical results within EMIS and take appropriate action for
any abnormal findings. They are also responsible for addressing any clinical queries arising from
virtual or face-to-face consultations.
In cases where a procedure is deemed necessary, the appropriate local clinical pathways will be
followed. This includes ensuring informed consent and conducting any necessary pre-operative
investigations.
Clinical sessions are structured for 4 hours and 10 minutes. Initial consultations and follow-up
consultations can be varied between 12- and 15-minute slots. Additional time may be allocated for
specific needs, such as using interpreters or supporting patients with additional requirements. Blocks
of time are built into each session to allow for catching up on delays, complex consultations,
technical issues with virtual consultations, and to ensure sufficient support is provided for any
queries.
At the end of each clinical session, admin time is dedicated for addressing results, preparing referral
letters, completing tasks, and ensuring that all documentation and follow-up actions are completed.
Other duties include:
- To provide the full range of NHS Primary Medical Services to the patients including clinical
examination, diagnosis, treatment and referral as appropriate.
- To be personally accountable for professional and ethical actions and practices within a legal and
ethical framework that adheres to the GMCs requirements and local policies and procedures.
- To ensure that prior to any course of action involving individuals/groups, their informed consent
or other valid authority has been obtained.
- To ensure that record-keeping is managed through the creation and maintenance of concise,
contemporaneous computerised records, and that all care is recorded accurately and in a timely
fashion. Notes should include a Problem READ coded, history, examination where appropriate
and a clear management plan.
- To interpret and explain complex facts in relation to a patients condition, taking appropriate
action and giving advice in a professional manner to team members, and when appropriate to
family and other carers, in such a way that protects and preserves the patients dignity.
- To have continuing responsibility for Adult and Child Safeguarding in all cases of suspected or
confirmed abuse, and to ensure compliance with local and national guidelines, policies and
procedures.
