An exciting opportunity for a Specialist Respiratory Clinician
(Physiotherapist or Nurse) has arisen in the Norfolk and Waveney (East Coast - Great Yarmouth and Waveney) ICS area. The key purpose of this role is to deliver an NHS
commissioned community pulmonary rehabilitation and Home Oxygen Assessment and Review (HOSAR) Service. This is a chance to be part of a team that
values quality improvement and innovation and strives for clinical excellence.
This is a development post ideal for someone wishing to progress from a Band 6 equivalent senior role to a specialist role in community respiratory care. Development is expected to take 12-18 months depending on the prior experience of the successful candidate.
Main duties of the job
The specialist Respiratory Physiotherapist / Nurse supports the Senior Team Lead to plan, promote, co-ordinate, develop, implement, and evaluate a high quality, evidence based Pulmonary Rehabilitation Service. The post-holder will be responsible for the day-to-day management of the pulmonary rehabilitation programme to ensure that it delivers pre- and post-assessment, exercise, and education to patients within a group or virtual setting and provides encouragement and support for patient self-management. The post-holder will also be expected to contribute to
delivery of the other services provided by the team, such as heart failure rehab, home oxygen assessment and review (HOSAR) and any other service
development projects.

Principal Responsibilities
- To undertake comprehensive subjective
and objective assessment of patients with diverse or complex respiratory
conditions and multiple pathologies. To prioritise patient problems and to
formulate and deliver appropriate treatment programmes based on assessment
findings, with evidence of discharge planning and in line with national
standards. This process is fully autonomous, and treatments will vary
according to assessment findings.
- To evaluate patient progress, reassess
and alter treatment programmes if required, based on knowledge of evidence-based
practice and patient outcomes.
Frequently readjust plans as situations change / arise due to
unforeseen circumstances.
- To lead pulmonary rehabilitation classes delivering a comprehensive programme of exercise and education. Including objectively measured assessment of exercise performance and exercise prescription and progression and assessment, interpretation, and feedback to the patient of initial and final outcome measures.
- Identify patients who meet the national criteria for home oxygen, provide specialist assessment and highly skilled
discussion of complex and highly emotive information to patients. Prescription of appropriate equipment
and flow rates will frequently require multifactorial decisions to be made
to individualise the prescription to optimise the patients concordance
and usage of the equipment.

demonstrate effective communication and negotiation skills across a range of
situations, both clinical and non-clinical

assess risk and manage it effectively within the clinical caseload and
community setting
- To contribute to the overall planning, delivery,
strategic development, and evaluation of the Pulmonary Rehabilitation and HOSAR Service

To
identify and propose service changes for own work area and participate in

To be responsible for maintaining accurate, up to date
and comprehensive patient treatment records (electronic or hand-written) in
