Job summary

The NHS commissioned services provided by the BOC Wigan Primary Care Respiratory Service (Wigan PCRS) respiratory team include:

Diagnostic Service for Asthma and Chronic Obstructive Pulmonary Disease (COPD)

Chronic Respiratory Disease Management

We will accept referrals from local clinicians for adult patients that are registered with a GP within the Wigan Borough Clinical Commissioning Group (CCG).

Wigan PCRS provides a quality assured diagnostic service which consists of;

Quality assured ARTP standard Spirometry

Fractional Exhaled Nitrous Oxide (FeNO) when appropriate

An assessment to ensure differential diagnosis have been ruled out supportsadultswho have been diagnosed with a respiratory condition.

Chronic Respiratory Disease Management (CRDM) provides the optimisation of treatment/therapies for Asthma and COPD in line with local, regional & national guidelines.

The patientwill be seen by a member of our respiratory specialist team who maybe a nurse or respiratory consultant.

The Wigan PCRS team of respiratory specialist nurses and local respiratory consultant.

Main duties of the job

Delivering all aspects of patient care and education to the highest standards.

Ensuring that patient care is assessed, planned, implemented and evaluated in accordance with required organisational and professional standards.

Ensuring working within relevant governance systems and processes and that they are robust and adhered to.

Supporting the workforce is fit to practice by offering additional support to those in primary care if required and also student nurses, medical students and trainee physician associates.

Assessing, diagnosing and making recommendations for treatment or prescribing effectively to improve patient outcomes

Working in partnership with patients to develop effective care plans

Wigan Primary Care Respiratory Service (Wigan PCRS) has been successfully delivered borough wide since April 2016. At present we are active in 89% of the boroughs GP surgeries.

From the outset Wigan PCRS has worked with and alongside, GP Practice staff to support in delivering the best possible health outcomes for patients with COPD and asthma across the CCG population.

Job responsibilities

The role of the Respiratory Nurse is to support the delivery of the objectives of the Wigan Borough NHS CCGs Primary Care Respiratory Service (PCRS), as part of a team of Respiratory Nurses.

The aim of the PCRS is to improve COPD and asthma patient outcomes across the borough and provide earlier, quality assured diagnosis and optimise treatment.

Respiratory Nurses will be flexible to work across all PCRS hubs within the borough to meet the Wigan PCRS service demand.

The post holder will:

  • Take a comprehensive history, and perform a systematic physical examination establishing baseline data to assess patients with COPD and asthma. Make use of support systems and work within agreed protocols and guidelines.
  • Perform quality-assured diagnostic spirometry as required- either to help establish a diagnosis or to monitor disease progression.
  • Analyse signs and symptoms, laboratory tests and other measures of function, in formulating a diagnosis.
  • Be responsible for providing cost-effective, co-ordinated and comprehensive clinical care based on the best current knowledge of treatment outcomes, with attention to safety, cost, invasiveness, simplicity, acceptability, adherence and efficacy.
  • Raise awareness and standards of respiratory care and management within primary-care settings.
  • Collect, collate and organise data on the health and well-being and related needs of patients within the caseload, drawing appropriate conclusions and taking appropriate action.
  • Collaborate with appropriate clinicians/healthcare professionals to plan and implement treatment for acute/chronic illness as required, using skills in nurse prescribing and medicines management.
  • Utilise advanced communication skills to facilitate the understanding and concordance of medicine management with patients and carers.
  • Respect the patients need for privacy and dignity.
  • Ensure clinical practice is carried out as directed by professional codes of practice and organisation policies.
  • Empower the patient by respecting patient values, setting lifestyle goals and providing information to support decisions about disease management.
  • Act as patient advocate to support and encourage the patient to identify sources of relevant information to assist with self-care and health promotion.

Education and Training

  • Participate in the ongoing development of a supportive learning environment.
  • Contribute to the informal teaching of support staff and students.
  • Participate in patient education and health promotion programmes.
  • Be responsible for the recognition of learning needs and contributing to learning outcomes of all levels of staff.
  • Act as a mentor, clinical supervisor, facilitator, educator and assessor for all pre and post registered nurses and allied health care professionals
  • Be responsible for creating an environment conductive to the acquisition of further knowledge and skills.
  • Contribute to the orientation of new staff
  • Liaise with all colleagues in a teaching capacity to ensure an overall co-ordinated approach for staff development.
  • Take responsibility for pursuing own professional development in accordance with an agreed personal development plan.

Service Development

  • Support the development and implementation of an effective specialist nurse-led service within the CCG in line with current best practice.
  • Contribute to increasing the prevalence of COPD and asthma through quality-assured spirometry.
  • Clearly articulate the role of the service and its value to patients to patient their carers and members of multidisciplinary teams.
  • Contribute in the development of strategies, protocols and policies that impact on the future service provision and care.
  • Network locally, regionally and nationally, recognising the ethical, legal and professional constraints of practice.
  • Contribute to the analysis and compilation of reports and presentations with regard to the service.
  • Act as a resource and role model for nurses and allied healthcare professionals.

Becoming a source of respiratory expertise to all healthcare professionals within Wigan Borough CCG, providing specialist respiratory training, advice and support as required.
