Job summary

We are looking to recruit a Head of Patient Safety to join our nursing leadership team within a 110-bed local independent secure Mental Health Hospital. Main duties of the job

  • The post holder is responsible for overseeing the organisation and management of patient safety incidents and investigations and the associated learning and improvement, and for feeding back outcomes to the relevant department.
  • Assist in the entry, updating and closure PSIIs onto the STEIS database and ensure this is regularly updated.
  • Contribute to the development and delivery of patient safety education and training programmes relating to incident reporting and management
  • Provide professional advice, guidance and support to managers and staff at all levels
  • Prepare and deliver teaching and presentation materials, relating to Quality, Patient Safety and Risk to meet the needs of staff across the organisation.
  • To lead a systematic approach to learning from incidents and support a Just Culture.
  • updating progress against Patient Safety Investigations and actions, providing updates as requested and updating progress on the internal and external systems in line with policy
  • To lead on learning from medication errors and support nurses who have made errors in practice.

Main Role Purpose

Responsibility for the development and implementation of therapeutic security practices within Cheswold Park Hospital and Associated premises. This specifically includes relational and procedural security.

To Lead on specific quality improvement work supporting the patient safety agenda to improve patient safety across all wards and premises associated with Cheswold Park Hospital

The post holder will work in conjunction with the Leadership team across the organisation to develop and provide specialist training to all clinical and non-clinical staff to further enhance the skills of the workforce in all areas of the Hospital and Associated premises

Main Duties/Responsibilities

In conjunction with the Chief Nurse lead on the implementation of the Patient Safety Strategy within Cheswold Park Hospital and associated premises

Provide expert advice and support to staff across Cheswold Park Hospital and associated premises on Patient Safety matters

Have oversight of compliance with Duty of Candour statutory requirements within Cheswold Park Hospital and associated premises

Promoting collaboration and transfer of best practice within the hospital ensuring lessons learned

Have oversight of and where appropriate, lead on incident de-briefs relevant to all patient safety and therapeutic security breaches

Lead on Ligature risk assessment across the Hospital and support wards to implement resultant action plans

Lead on Patient Safety & Clinical Security reporting processes for the Executive Leadership Team to ensure these are effective and promote a positive reporting culture within Cheswold Park Hospital and associated premises

Oversee the provision of patient safety data and information at an organisational level

Have oversight of the quality improvements that are ongoing in across the hospital and associated premises relating to all aspects of patient safety and clinical security

Lead on the provision of benchmarking submissions for patient safety

Lead on the development and review of all relevant policies within the time frames set

Advise Managers on the modification and improvement of therapeutic security practices using the results from clinical audit ensuring that the highest possible standards are maintained

To participate as a member of a multi-disciplinary team in initiating, implementing and evaluating operational policies and clinical practice against agreed criteria

Develop and Provide specialist training to staff and students in all aspects of Patient Safety and Clinical Security

Develop and deliver an induction training package for all new starters at Cheswold Park Hospital and associated premises on Patient Safety and Clinical Security

Contribute to the review and development of policies related to the role within Cheswold Park Hospital and associated premises

Undertake all mandatory training

The role can be physically demanding and there is a requirement for all ward-based staff to be able to undertake physical intervention training; and to be able to utilise these skills post training.

To support the Human Resources department with all investigations and disciplinaries relating to Patient Safety and Clinical Security and implement and lessons learned

Maintain a professional position at all times

Health and Safety

Attend all Health and Safety updates and training required as part of hospital statutory training.

Assist in the maintenance of physical security and safety through regular monitoring and checks.

Actively undertake responsibility for the Health, Safety and Welfare of him/herself and other employees in accordance with legislation and specific Policies and Procedures.
