Job summary

We have vacancies for qualified Pharmacists to work on our bank of Pharmacists to meet our service requirements.

You will be responsible for ensuring safe and effective delegated prescribing for Medefer patients. You will be expected to review our Consultants prescribing, undertake pharmacist clinical reviews and ensure each prescription is appropriate for the patient before completing a delegated prescription for them. The role is closely aligned to an outpatient pharmacy service within secondary care.

Main duties of the job

As a Pharmacist, you will:

  • Review our Consultants prescribing and complete pharmacist clinical reviews.
  • Contact patients by telephone to talk through prescriptions and advise on possible side effects or particulars of prescribed medication.
  • Review Consultants prescriptions, including a full clinical pharmacy check (including, but not limited to: allergies, dosage, frequency, formulations, interactions, appropriateness and more).
  • Provide detailed feedback to a Consultant should a prescription be deemed incorrect in any way, or utilise your competence to safely change the prescription, within reason.
  • Complete a final prescription for the patient remotely using remote prescribing software.

Hours:Flexible, on an ad hoc and casual basis

As a Pharmacist, you will:

  • Review our Consultants prescribing and complete pharmacist clinical reviews.
