Job summary

The post holder will be a member of the CARIAD Team within The Hospice of the Valleys and be a single point of access for short break requests within Torfaen and Blaenau Gwent.

To assess the needs of carers and provide a flexible short break service, arranged at the request of the carer including providing 1-1 sits and the coordination and facilitation of groups.

The aim of the CARIAD service is to provide high quality care for people with dementia and their families who are experiencing complex symptoms that are not being effectively managed by generalist practitioners. This work aims to enable them to remain in their identified preferred place of care, avoiding unnecessary admission to hospital.

The purpose of the AMSER project (this role) is to provide a flexible short break service, arranged at the request of the carer.

Support and facilitate choice and the fulfilment of the wishes of people with dementia and their carers.

Main duties of the job

To co-ordinate the promotion of the Your Time, Your Choice project within Torfaen through social media, printed literature at various outlets including associated groups and facilities, Memory Assessment Service Clinics and GP practices.

To engage with carers and asses their short breaks needs & suitability to the service using an agreed assessment referral form developed by a working group.

Notify Safeguarding officers should the carers needs require further intervention.

Initiate follow-up contact with the client following their short break provision to evaluate their experience.

To build a professional relationship with service users & their families and to use that relationship to safeguard and promote positive outcomes.

To update and maintain accurate records of service provision and clients details via Charity Log and other systems.

To utilise Charity log for scheduling of short breaks provision.

Prepare monthly reports & case studies to the CARIAD Team Lead in line with the outcomes & objectives of the project.

Prepare necessary reporting and statistical information for Carers Trust Wales.

To be able to work in the community in times of absence and provide cover for short breaks services, to ensure the smooth operation of the pilot.

Provide validation service and evaluation reporting of the project using the agreed evaluation method.

To attend supervisions, either on a one-to-one basis, or as part of a group in accordance with Hospice of the Valleys policy and guidelines.

About us

Hospice of the Valleys provides specialist palliative care to the people of Blaenau Gwent.

We work with people who have a life limiting condition such as heart failure, Chronic Obstructive Disease (COPD) and cancer.

Specialist Palliative Care helps patients and their families with physical, emotional, social and financial problems that are difficult to control. We can help to plan ahead, provide support at the end of life and through bereavement.

Patients may only need care from Hospice of the Valleys for a short time and can be discharged from us if their symptoms are controlled.

We also have a designated service for patients with Dementia and their carers called CARIAD (Care and Respect in Advanced Dementia).

Our team of professionals provide care for patients:

At home In their regular place of residence including nursing homes, Ysbyty Aneruin Bevan or Ysbyty Tri Chwm At our out-patient clinical in the Hospice in Ebbw Vale

We work closely together and alongside all of Health and Social Care providers including GPs, district nurses, hospital teams & voluntary workers. We place the patient and their family at the heart of everything we do.

Who we care for

Hospice of the Valleys cares for people living in Blaenau Gwent whose illnesses are no longer curable. We aim to care for and support people in the place of their choice, whether it is in their own homes, supported by our Hospice at Home and Community Teams, in our Out-Patient Clinic, or in the local Hospitals.
