Job summary
An exciting opportunity has arisen for an experienced Registered Mental Health Nurse to join our team, working 15hours per week. We are looking for a clinical professional with the ability to provide specialist support to patients and their families within our specialist dementia service.
Working within our CARIAD Team (Care and Respect in Advanced Dementia), the post holder will provide high quality palliative care for people with dementia and their families who are experiencing complex symptoms that are not being effectively managed by generalist practitioners. This work aims to enable them to remain in their identified preferred place of care, avoiding unnecessary admission to hospital. You will possess the drive and ambition to help shape our services to enable us to make a real difference to the lives of our patients and their families. The post holder will manage their own caseload.
Main duties of the job
To work autonomously, assessing patients and carers, with complex physical, emotional, social and psychological symptoms on a daily basis to enable an appropriate plan of care to be developed within the community, day centre or care home setting.
To provide leadership, support and give specialist clinical advice to health care professionals, patients and carers, concerning the care and treatment for palliative patients in all areas, hence ensuring effective decision making can be reached and appropriate treatment plans are developed. This will include frequently advising on medication, psychological issues and further treatments.
To establish and maintain effective communication relating to complex, sensitive and potentially stressful situations on a regular basis. Breaking bad news with patients, carers, relatives and other professionals throughout the patients illness. Supporting relatives, carers and staff in bereavement with the ability to recognise and refer on for complicated grief reactions.
To develop own and others knowledge and practice in order to influence clinical decision making across professional and organisational boundaries. To maintain accurate and contemporaneous records. To input and retrieve patient information from computer-based systems.
To use enhanced levels of both verbal and non-verbal communication and inter-personal skills with patients, relatives and carers, other multidisciplinary teams and multiple outside agencies.

Our specialist teams provided support to over 600 patients last year; in their own homes, in our Day Centre, at Outpatient appointments or in hospital. Contribute to the campaign to raise public awareness of dementia on matters relating to death and dying, supporting families to have difficult and sensitive discussions and address the issue of advanced care planning. Support and facilitate choice and the fulfilment of the wishes of people with dementia.
SCOPE AND RANGE -Carry a complex caseload and in doing so undertake the generic responsibilities required of all Registered Mental Health Nurses. Work in collaboration to deliver sustained improvements in performance, quality and consistency of practice across the team. Thereby achieving the best outcomes possible for adults living with dementia.
Develop innovative ways of working to ensure positive outcomes for those receiving the service. Represent and promote the service in relevant multi agency meetings;
