Job summary

We are seeking to appoint an enthusiastic and confident social worker to join our active and well established team on a fixed 2 year term contract - 15 hours per week (0.4 WTE). There may be an option to extend the contract subject to funding. You will be working as part of a multi-disciplinary team, providing a specialist palliative care social work service to Hospice patients, their families, carers and bereaved relatives. You will engage with colleagues, patients, patients families and carers, volunteers, supporters and visitors. You will contribute to effective multidisciplinary team working and discussion, actively contributing to the in house development of standards of care, audit tools and outcome measures related to the service. Work to maintain the values, mission and aims of Hospice of the Valleys.

Main duties of the job

You will support Hospice patients, families and carers with the practical and emotional implications of facing a life-limiting illness. Support Hospice patients, families and carers in making decisions about their future needs as they adjust to a new and changing situation, and offer appropriate interventions. This includes work with Hospice patients, families and carers coming to terms with the illness and possible death of someone close.

Maintain accurate and appropriate records, including the computerised database, and provide reports and statistics as required.

Evaluate the effectiveness of social work practice, and participate in multi-disciplinary audit.

Provide a key role in the development of clinical policies, protocols and guidelines in liaison with the Head of Clinical Services.

Use of a car and a full driving license is essential.

About us

Hospice of the Valleys is the specialist provider of palliative care services in Blaenau Gwent. Our specialist teams provided support to over 600 patients last year; in their own homes, in our Day Centre, at Outpatient appointments or in hospital. We provide nursing support, respite care, social work and bereavement support, physiotherapy, complementary therapies and welfare rights support to people with life-limiting illnesses across Blaenau Gwent. At present we also provide bereavement support in Merthyr on a temporary basis.
