West Kent Primary Care (WKPC) is seeking to employ an Enhanced Nurse on behalf of Weald Primary Care Network (PCN) to work across their member practices. The Enhanced Nurse will be responsible for managing and providing high-quality care for patients including residents within the local care homes.
Main duties of the job
Weald Primary Care Network (PCN) is looking for a caring, compassionate and experienced Enhanced Nurse with significant post-registration expertise in frailty. The Enhanced Nurse will be focusing on delivering high-quality, coordinated care to residents in care homes across the Weald locality. This will include providing care from initial history taking, clinical assessment, diagnosis, treatment, to a treatment plan. The post-holder will lead the development and delivery of enhanced nursing services within care homes, ensuring excellent management of patients with complex long-term conditions such as hypertension, diabetes, and respiratory diseases.
They will work within their professional boundaries and scope of practice within the primary care setting. The post holder will be required to communicate and work collaboratively with all stakeholders to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. They will focus on the four pillars of advanced practice: Clinical Practice, Leadership, Education, and Research.

About West Kent Primary Care
West Kent Primary Care is an innovative GP Federation whose shareholders include all 50 practices in West Kent, covering nearly 500,000 patients. Within this, the practices are divided into 9 Primary Care Networks (PCN). We are made up of a core operations team, a PCN based workforce made up of clinical and non-clinical staff and a Board of Executive Directors consisting of four local GPs and twelve PCN clinical directors. The federation also offers a variety of services including, but not limited to, the Community Diabetes Service, Post Covid Assessment Service, Chronic Fatigue Service and a Frailty Service.
