Job summary

Join West Kent Primary Care and become a vital member of our Tunbridge Wells Primary Care Network team as an Adult Mental Health Navigator. Tunbridge Wells PCN is made up of 8 member GP practices servicing over 80,000 patients. They offer PCN services ranging from First Contact Physio, Social Prescribers, Physician Associate and Clinical Pharmacy to make a range of services more accessible to our patients.

Main duties of the job

Social prescribing helps to strengthen personal and community resilience and reduce health inequalities by addressing the wider determinants of health. The Adult Mental Health Navigator will provide holistic support, connecting patients to local initiatives and services for practical and emotional support empowering them to take control of their own health and wellbeing. The Adult Mental Health Navigator role will work as part of the newly developed Mental Health Team providing support across the whole Primary Care Network and maintain a key role within the development of the PCNs Integrated Neighbourhood Team and champion multi-disciplinary working.

You will have the opportunity to join a monthly supervision group with other Social Prescribers and Health & Wellbeing Coaches from across West Kent for peer support and shared learning. After the completion of probation the Mental Health Navigator will be supported to enroll onto a Level 3 Social Prescribing Course.

Provide non-judgemental support and work from a strength-based approach focusing on a persons assets.

Take a holistic approach, based on the persons priorities and the wider determinants of health that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.

Work with individuals to co-produce a personalised support plan to address their health and wellbeing needs as required and access to local resources/services.

Conduct proactive wellbeing calls to those identified as being at additional risk of social and economic vulnerability.

Work with specific group of patients experiencing various mental health needs.

  • Educated to degree level or equivalent level,

  • Completed or working to complete recommended modules on Personalised Care Institute.

Experience

  • Previous experience working in the voluntary sector, primary care, population health, mental health, public health or health promotion.
  • Experience working directly with people including those with long term conditions, mental health issues or complex lives
  • Experience of a personalised approach to supporting people, coaching and motivational interviewing
  • Experience of developing and maintaining good working relationships with organisations and individuals
  • Experience of maintaining confidentiality
  • Experience of promoting and raising awareness of a service
  • Able to use Microsoft Office
  • applications with above average capability with Word, Excel and PowerPoint.
