become a vital member of our Tunbridge Wells Primary Care Network team as a PCN
Clinical Pharmacist. We are looking to expand our existing well established
clinic pharmacy team of 10 clinical pharmacists and 5 pharmacy technicians, by
recruiting a further 3 clinical pharmacists who are passionate about
developing their skills in General Practice.

Tunbridge Wells PCN comprises of 8 practices providing care
to approx 86,000 patients. Our pharmacists are given base practices in which
to work from. Our PCN team as a collective are proactive and innovative with
services being delivered by not only our large clinical pharmacy team but also
by first contact paramedics, first contact physiotherapists, Mental Health
Navigators, Care Co-ordinators, a dedicated pharmacy team for our local care
homes and more.
Main duties of the job
The pharmacists will develop, enable, and support a culture
of multidisciplinary care with a focus around developing PCN services in line
with national and local specifications. Working directly with various practice
teams; this role will directly link to colleagues in community and acute
settings ensuring patient health and outcomes are met in a collaborative and
innovative environment. The Pharmacist will support evening and weekend
extended access clinics working on a rota the other PCN staff. Our current set
up is one remote evening per week and one in six Saturday morning on site
clinics
The role will require a commitment to register on CPPE pathway and Independent Prescribers Course (if these havent previously been completed), which we will fully support you in by providing you with time to attend training sessions plus a GP clinical supervisor and peer support.
You will collaborate closely
with GPs and other Healthcare Professionals, conducting patient consultations
both via telephone and face to face, assisting with care plans for long term
condition management (including hypertension, cardiovascular disease, asthma,
diabetes and more), updating records, ensuring patient safety and managing
patients within the appropriate care setting plus undertaking audits to ensure
patient medication safety.
We are looking to recruit three full-time Clinical Pharmacists.
