Job summary

West Kent Primary Care is seeking to employ one enthusiastic and proactive pharmacist to join our established MDT Pharmacy Team in West Kent. The team work across the 9 Primary Care Networks (PCNs) in West Kent.

The post holder will directly improve patient care and support the development of care plans for patients with complex conditions. You will be a key member of the wider multi-disciplinary team working with practice teams and other professionals.

This post is integral to the NHS Plan supporting integrated care across general practice and the wider community services. The post holder will be encouraged to develop within the Primary Care setting and will partake in local national learning and development events as required.

This post is fixed term until 31 March 2025, with the possibility of extension.

Main duties of the job

You will cover a specific geographic area within West Kent, representing Primary Care in the MDT Teams and provide expert advice regarding medicines. You will be encouraged to develop your prescribing skills with complex patients. You will have input into the following areas:

MDT Meetings

Management of medicines following discharge to hospital for housebound patients

Conducting medication review and giving advice to patients and their carers

Management of patients with complex, long term illness as part of multidisciplinary services to housebound patients.

We seek individuals with good communication and interpersonal skills, and able to work as part of a team as well as autonomously.

It would be advantageous if you were an independent prescriber, but we will provide training and mentor support to become an independent prescriber.
