Job summary

In collaboration with system wide partners West Kent Primary Care have developed and continue to provide the Post Covid Assessment Service (PCAS) for patients across Kent & Medway.

The service is looking to recruit Band 6 registered nurses/health care professionals to join the team to assess, monitor, and support patients with Long Covid. The post holders will help deliver an educational programme as part of the handover of patients back to primary care.

This role will operate hybrid working. After an induction period and following sign off from the Lead Nurse the post holder will be able to work from home and our offices.

The contract is fixed term until 31 March 2025, with potential for extension.

Main duties of the job

In accordance with the local guidance, the post-holder will undertake clinical assessments and offer supporting advice, based on patient needs and the demands of the Service. The post holder will be aligned to the lead nurse to enable supervision and will gain advice from the wider multidisciplinary team.

Undertake virtual baseline assessment of patients clinical condition via agreed screening questionnaire ensuring completion to a satisfactory standard to allow MDT review and decision making.

Provide intervention and treatment advice to patients in line with service policy and using established clinical guidelines.

Educate and advise patients regarding assessment and management and when they should contact a doctor in line with guidance.
