Job summary

West Kent Primary Care (GP Federation) is looking to recruit an Advanced Nurse Practitioner (ANP) on behalf of Tonbridge Primary Care Network (PCN). Serving approximately 70,000 patients; Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices.

This role will support the PCN in increasing capacity patients have to access a clinical professional working across its member practices and care homes.

This is a 12 month fixed contract, with a veiw to extend.

Main duties of the job

Tonbridge PCN are seeking an Advanced Nurse Practitioner to complement their growing PCN team of Paramedics, Physician Associates, Pharmacists and Care Coordinators to support their practices in the delivery of urgent care. We are looking particularly for an ANP who is confident in triage and management of 'on the day' appointments. Communication with GP and MDT colleagues will be key in the ongoing follow up and treatment of patients as required.

The post holder communicates and works collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
