Job summary

In partnership with Maidstone and Tunbridge Wells NHS Trust, West Kent Primary Care (WKPC) has established a community diabetes service as part of an integrated community diabetes service for West Kent residents.

WKPC has an exciting opportunity to appoint a highly motivated forward-thinking Diabetic Specialist Nurse or Primary Care Nurse with a specialist interest in diabetes to join our team. There is an opportunity for the post holder to be full-time/part time. This is a permanent position.

The ideal candidate will have sound knowledge of diabetes and a willingness to undertake the non-medical prescribing course within 24 months of commencement in post and demonstrate a drive to continued learning.

Main duties of the job

The service operates clinics in local GP practices across West Kent, rotating between practices across Primary Care Networks (PCNs). The clinics are run by the diabetes specialist nursing team who work in collaboration with the practice nurses, supporting them to develop additional skills and competences to manage diabetes patients more independently.

The team is led by a highly skilled Diabetic specialist nurse with years of experience working in the Community who supports the team with ongoing clinical supervision and professional development.

We are extremely fortunate that our team is also supported by an external lead nurse Consultant who is renowned nationally in the field of diabetes who provides professional development as needed by the team.

In addition, to this support the team will be working alongside two local GPs with a specialist interest in diabetes who dedicate weekly sessions to champion the importance of proactive diabetes management in primary care.

Job responsibilities

  • The post holder will be responsible for the assessment, planning and provision of specialist diabetes nursing advice by delivering diabetes care to adult patients in local GP practices in West Kent via a visiting service.
  • Work closely with acute Trust colleagues to reduce the length of stay and help reduce readmission rates by organising appropriate follow up care upon discharge.
  • Work with acute Trust colleagues to avoid unnecessary admissions wherever possible by developing suitable management plans and providing patient education and support to effectively manage their own condition.
  • Deliver diabetes clinics in GP practices across West Kent through a combination of patient face to face and virtual clinics dependent on the needs of the patient.
  • The Diabetes Specialist Nurses will work with Practice Nurses to support the running of local nurse-led spoke clinics across the Primary care networks

Essential

  • Level 1 NMC Adult Registered Nurse
  • Relevant post registration qualification
  • One of the below qualifications as a minimum:-
  • Diabetes Diploma/Certificate
  • Teaching qualification or equivalent expert experience

Desirable

  • Mentorship qualification or willingness to undertake
  • Counselling qualification or willingness to undertake
  • Non-Medical Independent Prescriber/ Willingness to undertake.
  • DAFNE qualification

  • Post registration experience
