Job summary

West Kent Primary Care are looking to recruit a Health and Wellbeing Coach on behalf of Sevenoaks Primary Care Network (PCN). Serving approximately 84000 patients; Sevenoaks PCN aim to develop, deliver, and measure healthcare services across the population of Sevenoaks, Westerham, Edenbridge and Borough Green.

This role will be based within GP practices seeing patients for face-to-face appointments.Due to travel requirements for the role it would be beneficial for applicants to have a full driving license

Main duties of the job

We are looking for a creative and dynamic individual to support, encourage and motivate patients in adopting a healthier lifestyle approach and feel empowered to manage their health in the future.

A successful post holder will provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently through

Coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.

Providing interventions such as self-management education and peer support.

Supporting patients to establish and attain goals that are important to the patient.

Assist patients in building community resilience and make informed decisions and choices regarding their health and wellbeing.

Working in partnership with the Social Prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing.

Utilise health coaching skills to support to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.

Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their health and wellbeing and subsequently supporting them in shared decision-making conversations.

Essential

  • Experience of working in the health and wellbeing sector, ideally with a coaching qualification.

Desirable

  • Experience of working within a range of patient groups, including ethnic minority populations and various age ranges.
  • Understanding of relevant health conditions that may impact on a persons ability to adopt healthy lifestyle choices.
  • Understanding of behavioural change techniques.
  • Knowledge of mental health and general wellbeing
  • Knowledge of obesity management, general dietetics, weight management, nutrition, and physical activity in relation to management of long-term conditions and health improvement.
  • Understanding of health inequalities and how these impact on population health outcomes.
  • Experience of multi-disciplinary working and in partnership with other health professionals or teams.

Qualifications

Essential

  • Is enrolled in, undertaking, or qualified from appropriate health coaching training, with the training delivered by a training organisation listed by the Personalised Care Institute.

IT Skills

Desirable

  • Competent IT skills, familiar with clinical systems: Vision, Emis, Docman. Able to use Microsoft Office applications: Word, Excel, PowerPoint.
