Job summary
This post offers a unique opportunity to work between primary and
community care, focusing on the management of patients with frailty whilst
offering exposure to a range of general medicine and general practice within
primary care and community virtual ward beds.
This
rotational post will incorporate time working with Tonbridge Primary Care Network (PCN) for 3 days a
week, and 1 long day working in the Frailty Virtual Ward provided by the West
Kent Home Treatment Service at Kent Community Health Foundation Trust (KCHFT).
The post holder will be employed by West Kent Primary Care on behalf of the Primary Care Network (PCN)
and will be seconded to the KCHFT for part of their working week. As part of
the role the post holder must be willing to take part in the First Contact
Practitioner Roadmap with Health Education England.
Main duties of the job
The Paramedic Practitioner will work closely with GPs within the
growing PCN frailty MDT; which includes GPs, care home pharmacists, care home
coordinator, community nursing teams and therapists. The role will primarily support patients who are housebound
and living in care homes and may include some outreach work with patients who
struggle to engage with medical services, aiming to reduce health inequalities.
As a member of the Home Treatment Service clinical team, the Paramedic
Practitioner will work with acute frailty services. They will provide
assessment, diagnosis, treatment planning and implementation of management plans
for patients on the frailty virtual ward discharged from hospital or referred
by GPs or the community teams.
