Job summary

An exciting opportunity has arisen for registered nurses or health care professionals to join the established Post Covid Assessment Service (PCAS) for Kent and Medway working with in a multidisciplinary team.

You will support patients during their recovery from Long Covid using remote monitoring technology and implementing the use of the ‘Your Covid Recovery’ app. Working through Multi-Disciplinary Team Meetings you will be involved with other professionals; like GPs, mental health nurses and physiotherapists, in the decision-making regarding ongoing support for individuals with Long COVID symptoms.

This role is fixed term until End March 2023, with view to extend.

Main duties of the job

The post holders will develop good working relationships with both the primary care team and wider healthcare professionals from other organisations and will work within local guidelines relating to provision of post covid services.

In accordance with the local guidance, the post-holders will undertake clinical assessments and offer supporting advice, based on patient needs and the demands of the service. The post holders will be aligned to the lead nurse and service manager to enable supervision and will gain advice from the wider multidisciplinary team.

West Kent Primary Care is a rapidly growing GP Federation whose shareholders include all 54 practices in West Kent, covering nearly 500,000 patients. Within this, we are divided into 9 Primary Care Networks (PCNs), each with its own leadership structure. West Kent Primary Care is made up of an office-based team, clinical staff and clinical and administrative staff employed on behalf of the PCNs.

JOB PURPOSE

In collaboration with the PCNs & wider system partners West Kent Primary Care have developed and continue to provide the Post Covid Assessment Service (PCAS). Working as part of a multi-disciplinary team the post holder will assess, monitor, and support patients virtually.

The post holder will report to the Lead Nurse and their line manager at West Kent Primary Care. They will develop good working relationships with both the primary care team and healthcare professionals from other organisations and will work within local guidelines relating to provision of post covid services.

KEY RESPONSIBILITIES

In accordance with the local guidance, the post-holder will undertake clinical assessments and offer supporting advice, based on patient needs and the demands of the Service. The post holder will be aligned to the lead nurse to enable supervision and will gain advice from the wider multidisciplinary team.

The postholder will be required to:

Day to day management of the assessment & triage hub in the absence of the Lead Nurse including responsibility for co-ordination of the MDT and hub team.

Undertake virtual baseline assessment of patient’s clinical condition via agreed screening questionnaire ensuring completion to a satisfactory standard to allow MDT review and decision making.

Onboard patients onto the ‘Your Covid Recovery’ App.

Monitor patents virtually through the dashboards – including review of data entered by the patients and providing advice to the patient or escalation as required on a weekly basis.

Support colleagues within PCAS to complete tasks efficiently and effectively as directed by the senior team.

Follow agreed clinical protocols referring to senior colleagues or GPs as appropriate.

Collate, analyse, and present clinical data and information to the team using appropriate charts and/or graphs to enhance care.

Provide interventions and treatment to patients in line with service policy and using established clinical guidelines.

Educates and advises patients regarding assessment and management and when they should contact a doctor in line with guidance.

Makes onward referrals as required.

Ensure infection, prevention and control guidance is applied in practice.

Ensure equipment is available for patients who require virtual monitoring in their homes.

Fully document all aspects of patient care and complete all required paperwork for legal, professional, and administrative purposes.

Monitor and support junior members of the team to complete all required paperwork.

Record clear and contemporaneous IT based consultation notes to agreed standards.

Maintain up to date knowledge of clinical guidance i.e. local guidelines, policies and procedures.

Other responsibilities

Deputises for the Lead Nurse where required.

Provide a leadership style which is underpinned by strongly held values around equality, diversity and openness.

Manages disposable stock levels and equipment and reports deficits or shortages to the Lead Nurse.

Ensures the privacy and dignity of all patients is maintained.

Enable others to promote equality and diversity in a non-discriminatory culture.

Supporting colleagues and responding to requests from administrative and healthcare professionals, working effectively within the wider team.

Takes responsibility for ensuring compliance with all relevant practice policies/guidelines, e.g. safeguarding, resuscitation, confidentiality, data protection, health and safety.

Works in accordance with the professional guidance provided by the NMC.

Training and Development
