Job summary

This Paramedic Practitioner role will be based in the Weald Primary Care Network area.

West Kent Health GP Federation is seeking to employ an enthusiastic and forward thinking Paramedic Practitioner on behalf of Weald Primary Care Network. West Kent Federation as an established employer and provider will act as the lead employer and support your work within the PCNs.

Main duties of the job

We are seeking an individual to develop, enable and support a culture of multidisciplinary care within PCN services in line with local and national specifications. This role will underpin Enhanced Health in Care Homes whilst also supporting the management of housebound patients. Working directly with various Practice teams; this role will directly link to colleagues in community and acute settings ensuring patient health and outcomes are determinants of treatment in a collaborative and innovative environment.

Job description

Job responsibilities

The key responsibilities:

To assess, diagnose, treat, refer or signpost patients/service users who require a visit with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

Use advanced clinical skills to provide education to patients, promoting self-care and empower them to make informed choices about their treatment.

Service development, transformation and improvement

Proactively develop MDT meetings to discuss frequent service users in order to put comprehensive care packages in place that lead to more optimal use of services and improved care.

Mentorship and supervision of junior colleagues and peers, including train the trainer courses to skill-up other members of the team and partner organisations.

Be open to learning from other professionals including General Practice and Nursing to expand the scope of your own practice and holistic care.

Clinical Responsibilities

To provide high quality holistic patient care and treatment.

Ensure that professional standards are maintained and within the guidance by the Department of Health, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP)

Supply medications as appropriate following policy, patient group directives and local pathways (Qualification dependent)

To advise and support junior members of the team in the care, treatment and anticipatory care planning for patients.

Participate in setting standards for care and auditing clinical outcomes.

Identify and complete necessary interventions, within scope of practice and following guidance from existing policy and procedures.

When necessary refer to facilitate further advanced assessment and guidance from the wider teams.

Work closely with other care providers and agencies to ensure quality and safety.

Refer all safeguarding concerns, using established safeguarding referral pathways.

Accurately and professionally document all patient contacts, following all relevant organisational policy and procedure. This will include at least two different clinical systems.

Know own limits in relation to scope of practice and recognise when a requirement exists to refer to or seek advice from other clinicians.

Take responsibility for the safe storage and administration of drugs and equipment.

Participate in setting and monitoring standards to ensure effective care and delivery of evidence based practice.

  • Minimum 3 years post qualified experience
