Job summary
Newham Health Collaborative is seeking more
clinical pharmacists to work within Newham PCNs. Our clinical pharmacists work with named
GP practices within Primary Care Networks, providing buddying and peer-support
arrangements to each other. As well as enrolment in national mandated training
pathways, our pharmacists also engage with an inter-professional training/supervision
programme, which focuses on local priorities and individual learning needs.
Pharmacists looking for part time roles are also welcome to apply.

Main duties of the job
The
post holder will provide expertise in clinical pharmacy and medicines
optimisation as part of a multi-disciplinary team providing care for patients
from general practices. You will be responsible for delivering care directly to
patients; optimising medicines for patients with chronic disease as well as
responding to medicines-related queries and undertaking medicines
reconciliation. You will lead quality improvement projects and provide
leadership for medicines use to ensure patients experience high quality primary
care. You need to be an excellent communicator with a proven ability to work
with other health care professionals across different care settings. You will
be supported to complete the independent prescribing qualification and to
participate in peer-to-peer group learning.

Newham
Health Collaborative(NHC) CIC is a GP federation representing all the GP
practices within the Newham borough boundaries. This covers a population of
approximately 400,000 patients. We are established as a Community Interest
Company, wholly owned by our 43 GP practice members. We invest any surplus we
secure into services for primary care and our patients.

Primary care clinical pharmacy is developing, and we are looking for a motivated individual who is flexible to respond to the developing nature of the role and the needs and priorities of individuals requiring support. Under the direction of the Senior Clinical Pharmacists and Clinical Directors we expect that you will be able to work independently and manage your own workload. You will need excellent communication skills to be able to liaise with patients, primary care staff, community pharmacists and other partners.
Hours of work should reflect general practice hours 37.5 hours per week within 8.00 am to 6.30pm. Job-share and flexible hours will be considered.
