Job summary

Newham Health Collaborative (NHC) is looking for a Part-time Primary Care Network Manager, to provide support for Docklands PCN, to enable the delivery of services in line with the Network Contract Directed Enhanced Service specification (PCN DES) and facilitate development of the PCN.

Main duties of the job

The post holder will develop strong relationships and engage with a wide range of stakeholders across the locality, to ensure collaborative approaches to service development and delivery.

The post holder will also provide management support for the Primary Care Network to enable the delivery of services in line with the Network Contract Directed Enhanced Service specification (PCN DES) and facilitate development of the PCN.

The PCN manager will manage the Network Additional Roles staff including the Clinical Pharmacist, Social Prescriber, care coordinators and any other roles established on behalf of the Network under the Additional Roles Reimbursement Scheme and other roles.

To achieve the above roles, the post holder will report to, work closely with and also provide management support to the Networks Clinical Director.

The post holder will also ensure the collection of appropriate data and information to ensure robust budget monitoring, oversight of payments and provision of assurance to the network and where required to commissioners regarding the Networks funds.

About us

Newham Health Collaborative(NHC) CIC is a GP federation representing all the GP practices within the Newham borough boundaries. This covers a population of approximately 400,000 patients. We are established as a Community Interest Company, wholly owned by our 43 GP practice members. We invest any surplus we secure into services for primary care and our patients.

Primary Care Networks will also be expected to think about the wider health of their population, taking a proactive approach to managing population health and assessing the needs of their local population to identify people who would benefit from targeted, proactive support.

Although Covid vaccinations are no longer a mandatory requirement within the NHS, NHC continue to encourage and support the vaccination uptake as this remains the best line of defence against Covid19. The safety of our staff and Newhams patients is a top priority for us at NHC.
