Newham Health Collaborative is looking to recruit an experienced, enthusiastic and motivated Senior Clinical Pharmacist to work at Tollgate Medical Centre within Docklands PCN. If you are committed to continually developing your knowledge and skills to improve patient care in General Practice, this is the role for you. We are looking to utilise and integrate the clinical skills of the additional Pharmacist into our General Practice/PCN team.
This is an exciting opportunity to be part of an innovative, forward thinking General Practice who has gained national recognition for their development of online triage in primary care. This resulted in a visit from the Prime Minister, Boris Johnson and being shortlisted as finalists for the HSJ Awards and GP Team of the Year in the GP Practice Awards. We continue to develop our pioneering work and we are looking for a dynamic individual to join or team.

Main duties of the job
Qualifications & Experience for the role:
Minimum of 3 years post registration experience, with General Practice experience
Certificate in Non-medical prescribing
Evidence of further education in clinical pharmacy is essential e.g. Completion of NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma (to apply for exemption from the NHS pathway training)
The post holder will ideally have have experience of leading and managing pharmacists.
Ability to competently use a number of IT systems eg. Microsoft office, EMIS, PINCER

About us
This role is supported by Newham Health Collaborative who recruits,
employs, and supports pharmacists working in General Practice to deliver high
quality patient services. We are a GP federation representing all the GP
practices within the Newham borough boundaries. This covers a population of
approximately 400,000 patients. We are established as a limited company, wholly
owned by our 45 GP practice members.
Since 1 July 2019, all the GP
practices in Newham have come together in ten Primary Care Networks (PCNs).
Although Covid vaccinations are no longer a mandatory requirement within the NHS, NHC continue to encourage and support the vaccination uptake as this remains the best line of defence against Covid19. The safety of our staff and Newhams patients is a top priority for us at NHC

post holder is an experienced pharmacist who would work with other pharmacists
at the Tollgate Medical Centre and the PCN, as well as have mentoring
responsibilities. The post holder will work as part of a multi-disciplinary
team in a patient-facing role. The post holder will take responsibility for areas
of chronic disease management within the practice and undertake clinical
medication reviews to proactively manage patients with complex conditions.
The
post holder will provide primary support to general practice staff with regards
to prescription and medication queries. They will deal with acute prescription
requests, medicines reconciliation on transfer of care and systems for safer
prescribing, providing expertise in clinical medicines advice while addressing
both public and social care needs of the patient.
The
post holder will provide clinical leadership on medicines optimisation and
quality improvement and manage some aspects of the quality and outcomes
framework and enhanced services. The post holder will ensure that the practice
integrates with community and hospital pharmacy to help utilise skill mix,
improve patient outcomes, ensure better access to healthcare and help manage
workload. The role is pivotal to improving the quality of care and operational
efficiencies so requires motivation and passion to deliver excellent service
within general practice.
