An exciting opportunity has arisen for a Transformation Manager, to join us at Newham Health Collaborative.
The Primary
Care Transformation Manager will have responsibility for leading on PCN and
Primary Care Support projects that are designed to ensure that service provided
by the host organisation (on behalf of PCNs) are – well led, responsive, safe
and effective in order to:

Improve clinical outcomes for the
population, with a focus on those with complex needs and long terms conditions.
·
Provide investment to primary care to
achieve the improved outcomes relating to patients needs
The
role will oversee the delivery of services which NHC, in collaboration with PCNs, deem appropriate for the fulfilment of
the above, and any funding allocated to the Provider will have to be used in
accordance to a developed service specification. Those specification may be for
PCNs, Federations, and/or their constituent practices, both within the borough
and/or across the entirety of TNW and
to NEL as appropriate.

Main duties of the job
The Primary Care Transformation Manager will be responsible for the transformational development of Primary Care Networks and Primary Care Support services in Newham.
Ensuring
that that local services especially PCNs are fit for the
future and provide best care for the needs of local community, the Primary Care
Transformation Manager will; oversee the delivery of key service
transformational work strands in primary care services ensuring alignment with
NHS planning requirements, local health and care priorities and commissioning
intentions, and the NHS Long Term Plan.
The Primary
Care Transformation Manager will provide leadership to PCN managers and in
collaboration develop, design and implement PCN service improvements and associated organisational development and learning programmes to ensure
the primary care and the PCN ARRS workforce is able to contribute, fully
utilised and is fit for future requirements.
With the ability to build strong working relationships
with Practices and PCNs, partners and key stakeholders the Primary Care Transformation
Manager will be able to communicate effectively to a wide range of stakeholders
both verbally and in writing in order to provide updates and assurance
regarding service improvements that are being delivered and implemented within
agreed timeframes.

Newham Health Collaborative Ltd (NHC) is a GP federation representing all the GP practices within the Newham borough boundaries. This covers a population of approximately 400,000 patients. We are established as a limited company, wholly owned by our 45 GP practice members.

- Undertake (as required) ad-hoc
pieces of work that are critical to the Primary Care Services agenda.
- Develop and analyse highly complex data sets, using national
and local standards and guidance as required, collate qualitative and
quantitative information to produce robust high value business cases and project
plans, that support decision making and service redesign that meets key
performance indicators and offers value for money.
- Sourcing and using benchmarking
and comparison data to influence decisions relating to; models of care,
business cases and strategic plans etc.
- To ensure that all records and
information are maintained in a way that allows up to date and timely
information to be available as and when required. Providing assurance on
the delivery of project deliverables. Through the production of highlight
reports and various management reports for NHC, as well as for partners
and relevant stakeholders.
- Support collaborative/
partnership working across Newham and with service users and carers and
other key local health and care organisations; including across NEL ICS,
NHS England and local healthcare providers. Ensuring that positive working
relationships with managers and staff are also developed.
- Work closely with NHCs Clinical
and Operations leadsas well as Commissioners and other key system stakeholders
to create an integrated approach to service transformation and business
development.
- Line manage and supervise
allocated member/s of staff and integrated care teams as necessary.
- To maintain robust project
monitoring and control mechanisms to ensure that projects can be
successfully managed and deliverables are met as a whole.
- Ensure that all project
activities and events such as meetings, teleconferences, conferences, and
learning events are planned, resources made available and work is
undertaken in accordance with the agreed Project Plan key milestones.
- To ensure that good practice in
project management as set out in PRINCE ( or equivalent methodology )is
adhered to and embedded – this will include the need to keep records of
discussions and progress and follow up actions.
- To lead on the participation in
research programmes associated with the areas of work outlined and agreed.
- To be responsible for
self-organisation/time management and able to plan and manage time
efficiently.
- Project monitoring and
reporting in line with NHC’s governance processes.
- Responsible for training in new
integrated systems, software and roll out.
- Travel across multiple sites
and care settings within Newham.
Communication
- The postholder will be required
to have regular contact with internal and external partners and
stakeholders and will often need to engage with them over sensitive,
highly complex, contentious and confidential issues.

contact on all significant matters relating to Transformation projects.
Being a spokesperson for projects and ensuring that all key individuals
are effectively briefed.
- Secure service user, carer,
staff and public voices in all elements of the work stream and ensure that
the principles of co-production, as an equal and reciprocal way of
working, are adhered to consistently throughout the duration of projects.
- Work closely with NHC’s SLT and
the Communication lead, preparing
and overseeing the development and dissemination of staff and public
facing communications.
