We are
looking for an energetic and enthusiastic Primary Care Network Manager to join
our developing Primary Care Network in Docklands, Newham. This exciting opportunity is available for
an enthusiastic and motivated leader to join as a Primary Care Network (PCN)
Manager.
We are looking for a solid operational manager who
is comfortable with entrepreneurialism, who is familiar with NHS commissioners
and providers. The role involves coordinating internal resources and external
stakeholders for the flawless design and execution of the portfolio of clinical
projects and services being undertaken by the PCN.
Hours of work: 2.5 days (18.75 hours) per week

Main duties of the job
The
appointee will play a lead role in working in a complex environment on a range
of business, transformation and improvement initiatives across the primary care
locality.
The
role will provide strategic transformation support and operational leadership.
The ideal candidate will be an experienced manager with a Primary Care
background to take the lead in understanding national and local requirements,
facilitating engagement between PCN member practices and implementing agreed
plans. PCN manager needs to work with
member practices to break down challenges to implementation across the PCN
population.
The
PCN Manager will develop strong relationships and engage with a wide range of
stakeholders across the locality, to ensure collaborative approaches to service
development and delivery.
About us
Newham Health Collaborative Ltd (NHC)
is a GP federation representing all the GP practices within the Newham borough
boundaries. This covers a population of approximately 400,000 patients. We are
established as a limited company, wholly owned by our 47 GP practice members.
Since 1 July 2019, all the GP
practices in Newham have come together in ten Primary Care Networks (PCNs)
covering populations of approximately 400,000 patients. These will also be the
footprint around which integrated community-based teams will develop, and community
and mental health services will be expected to configure their services around
PCN boundaries.
Primary Care Networks will
also be expected to think about the wider health of their population, taking a
proactive approach to managing population health and, from 2020/21, assessing
the needs of their local population to identify people who would benefit from
targeted, proactive support.
