Job summary
Newham Health Collaborative has a number of
vacancies for Care Coordinators within Docklands Primary Care Network. We are
looking to recruit Care Coordinators who are available to work on full time
and/or part time basis with flexibility when required to work across sites
within our PCN.
Docklands PCN is made up of 3 member practices located in the
Beckton and North Woolwich area. We work collaboratively as a Primary Care
Network (PCN), to pool our resources and workforce with the aim of providing
excellent integrated services for our patients.
The practices within Docklands
PCN cover a patient population of just over 43,000 patients. We are looking to
provide services for our patients that are accessible within the area that they
live.

Main duties of the job
The Care
Coordinators will ideally have A-level / NVQ 3 or equivalent experience in
admin / business / marketing / customer service environment.
The post holders
will have experience of working within healthcare, Primary Care Network would
be advantageous but not essential. You will have strong listening and
communication skills and will be familiar on local resources and services and
how to access them.
You will also have
the skills to deliver person-centred support in a non-judgmental way by answering
queries, making and managing appointments, utilising IT systems, and ensuring
that people have good quality written or verbal information to help them make
choices about their care.
Excellent patient
care is the core of our services, we will therefore require the post holder to
have excellent interpersonal skills as the role will be GP patient facing as
well as working with the multi-disciplinary team.

NHC is small limited shareholding company working with all 48 GP
practice members across Newham. Providing NHS Services based in Primary Care,
pooling the skills and resources of local GPs to provide large scale services
as part of the local NHS Strategy to bring more services into the Community to

professionals within the PCN to identify and manage a caseload of patients.
In collaboration with clinicians, management, administrative and
reception team colleagues, the Care Coordinator will help patients with complex
needs navigate the health and care system. This includes helping them to manage
their needs through answering queries, making and managing appointments, and
ensuring that people have good quality written or verbal information to help
them make choices about their care.
Work within Docklands PCN GP Practices to provide a central
co-ordination role for patient care planning. The role will be GP patient facing,
with the core responsibility being excellent patient care.
Co-ordinate care packages for patients as identified by the GP
across health, social care and mental health as appropriate, providing a
single-point of access for staff & service users, actively managing
patients care plan delivery
Facilitate smooth and planned discharge and handover between care
settings across the health and social care system, including GP, acute,
community, and be responsible for facilitating inter-agency communication and
support
Identify and work with a list of named patients with the aim of
encouraging independence, enabling people to remain at home, reducing
unnecessary admissions to hospitals and supporting early discharge from
hospital, improving the quality of care. Work as essential team member within the
practice and PCN.
Provide feedback to the practices, troubleshoot and escalate
actions as necessary, providing advocacy for service users.
