Job summary
This is a super opportunity for a self-motivated team player to join a team supporting General Practice in Newham. The role involves coordinating and providing administrative support to the multi-disciplinary team (MDT) meetings taking place monthly in each GP practice. The work includes the preparation of agendas and minutes, taking appropriate follow-up action as required. You will be working for Newham Health Collaborative, which is Newhams GP federation, and in delivering this service, you will be working with colleagues across several health and care organisations. The role requires great listening and writing skills, good relationship skills, initiative to suggest what can be improved and maturity to work appropriately with colleagues in other organisations. Above all it requires an interest in patients and patient care, and, for the right candidate offers the opportunity to grow your own skills, knowledge, and experience.
Main duties of the job
You will be working as part of a small team of committed, knowledgeable individuals, overseeing the quality of their work and supporting their development. You will also be each providing administrative support to GP multi-disciplinary team (MDT) meetings. This will include minute-taking, collecting patient information, organising diaries, working with colleagues to ensure all the information needed is available; in short, doing everything you can to ensure an efficient and productive meeting, that can lead to good outcomes for our patients. You will be a team-player, with the ability to learn quickly and on your own initiative. You must be proficient in Microsoft Office, especially using Outlook (email and calendar invites), Excel, and Microsoft Word. You must also be confident with computers as the role will include learning new specialist software such as GP Emis, and Webex. Personal responsibility, independence and pride in your work, and a sense of curiosity, will be an advantage.

Newham Health Collaborative:
Newham Health Collaborative Ltd (NHC) is a GP federation representing all the GP practices within the Newham borough boundaries. This covers a population of approximately 400,000 patients. We are established as a limited company, wholly owned by our 47 GP practice members.
Background:
The purpose of the Multi-Disciplinary Team (MDT) meeting is to support vulnerable patients with complex needs by bringing together different professionals to promote proactive planning and holistic, joined-up patient care. An MDT meeting (known as MDM) takes place monthly in every General Practice in Newham.
Primary, community, mental health and social care professionals review patients at the MDM and a plan of action is made, setting goals and agreeing care and crisis plans, increasing overall communication and helping timely shared decision making, ensuring the patients wishes are known and their needs met.
Key objectives of the MDT are to:
Work closely with providers to deliver joint health and social care, promote and mobilise care outside of hospitals as far as clinically safe and possible.
- Ensure the patient and their needs are always at the centre of the discussions.
- Improve communication and liaison between different practitioners and services, internally and externally, to ensure effective clinical care decision-making which are patient centred and evidence-based (in line with standards and NICE guidelines).
- Improve the journey and experience for the patient by considering a range of interventions and facilitating care coordination across a range of service providers as appropriate.
- To reduce preventable hospital admissions and reduce clinical delays/duplication across pathways.
- To ensure that safeguarding is routinely discussed and concerns escalated and followed through.
Main Duties and Responsibilities
Provide specialist administration support and advice to the MDT by:
Liaising with GPs, Nursing staff, Secretaries, Social Workers and other MDT members in preparing for the MDT. This will include working with GP practices, London Borough of Newham, East London Foundation Trust, Barts Health Trust and any other partner organisations.
Coordinating with the GP and MDT members the cases to be discussed at the meeting and communicating the list at least 3 days in advance of the MDT meeting.
Ensuring the relevant patients notes are accessed via EMIS and are ready for the MDT meetings.
Ensuring the EMIS MDT template is completed accurately and fully during the meeting or as soon as possible after the meeting.
Ensuring that any patient on the MDT list who is not discussed because of lack of time or inadequate information will automatically be added to the following meetings lists.
Typing up meeting notes/actions promptly.
Attending any NHC MDT CCG related meeting as requested by management.
Organising MDT meetings by booking rooms, circulating agenda and minutes and chasing actions.
Working closely with practices e.g. good communication, offering support to improve the quality of MDT meetings and reducing the risk of cancelations
Setting up and clearing up the meeting room before and after meeting.
Travel to and from meetings as required.
Setting up virtual video conferences and supporting colleagues with how to use them where necessary.
Dealing with telephone messages using own initiative to achieve best outcome.
Using Outlook Calendar for sending out meeting dates, including arranging and changing appointments, prioritising these as appropriate.
Undertaking administrative duties such as photocopying, diary coordination, and emailing.
Ensuring all urgent and/or confidential communications are received and distributed from/to relevant parties in a timely manner.
Inputting, monitoring and checking EMIS is updated and accurate.
Working with all team members to collect information for MDT performance reporting.
Acting as a point of contact for the teams, dealing and responding effectively with complex queries from any stakeholders and passing on relevant information to appropriate team members sensitively and autonomously.
Running and collating reports which may include reports to the CCG and NHC as required.
Developing and maintaining effective electronic and paper filing systems, to ensure that information is kept securely.
Working together with other staff within NHC, practice admin, ELFT, LBN, CCG and other providers to provide an effective network of communication and partnership work.
Carrying out other appropriate delegated duties as required by line manager
