Job summary
The role will work within an integrated care system to organise and manage up to 3 Neighbourhoods in a defined locality in Hillingdon (North, Central or South). Using a change management approach and methodology to identify, plan and deliver transformation initiatives to support and enhance Neighbourhood working.
Main duties of the job
Proactively engage staff within the Neighbourhood teamsand wider stakeholdersto enable delivery of initiatives
Committed to working and engaging constructively with internal and external stakeholders on a range of business sensitive issues
Foster and maintain strong links with all services across the PCN and neighbouring networks.
To nurture key relationships and maintain networks internally and externally.
Close liaison with the Communications and Stakeholder engagement team on public relations and marketing activities
Link with managers and members of other functions, to address inter-dependencies and ensure alignment

Overall responsibility for the Development and Delivery Plan for the assigned Neighbourhood/PCNs/Practices within your remit
Senior member of The Neighbourhood Management Team who will be responsible and accountable for the design and implementation of the Neighbourhood strategy and development
Working on various HHCP programmes within the Neighbourhoods and individual Neighbourhoods Business and organisational development plans.
Working on various Primary Care Network(PCN) programmes that support Neighbourhood working
Working with practices within the PCNs to support their development plan
Using population health data and various BI data to analyse individual neighbourhood development areas and to drive transformation and continuous improvements
Prioritise initiatives in complex and nuanced environments
Develop business cases and development proposals
Facilitate and enable transformational change - build excellent functional relationships with and between multiple stakeholders
Identify process improvement and where possible, streamlined ways of working
Utilise project management and change management methodologies to manage and deliver against the key deliverables (or key performance indicators KPIs)within the project to ensure it is successful
To achieve Neighbourhood objectives through effective facilitative partnership working and engagement to ensure change is embedded
Develop and maintain a collaborative, constructive, delivery focused culture across and between sovereign organisations, without compromising those organisations sense of their own autonomy
To investigate best practice elsewhere, evaluate applicability to the Neighbourhoods and develop plans to ensure successful implementation.
To identify areas of opportunity and duplication to maximise efficiencies in patient or service outcomes.
