Job summary

Hillingdon Health and Care Partners is focused on delivering integrated care across its providers with a wide range of partner organisations and stakeholders, including acute trusts, mental health trusts, local authorities, community services and primary care, as well as voluntary sector organisations.

The Care Home Support Teams has been established to provide holistic care and integrated care planning (ICP) to the residents of Older Adult and Learning Disability and Mental Health Care Homes within the borough. A “whole systems” approach will aide a reducing in hospital admissions and allow for patients to be cared for in their own homes where possible.

Main duties of the job

The MDT Care Coordinator will play a key coordinating role at the heart of the multi-disciplinary Care Home Support Teams and will help in coordinating care for our Care Home patient population to ensure interventions and care needed is provided in a timely manner. As part of the Service Administration team there will be an expectation that this role will provide support to the wider team as and when required and vice versa.

Job responsibilities

Operational Duties

• Ensure all case summaries are received from the Care Home Support Team for any residents that need to be discussed at the MDT meeting.

• Ensure all appropriate individuals are invited to the MDT as directed by the Care Home Matron; this may include other professional, Care Home Staff and Voluntary Sector colleagues.

• Ensure all members of the MDT panel have copies of the residents’ case summaries in a timely manner.

• Provide administrative support at the MDT meetings ensuring accurate minutes are produced which reflect the clinical conversations and that an action log is produced and followed up appropriate. This should also be circulated to attendees in a timely manner.

• To be responsible for arranging appointments and referrals agreed at the MDT meeting.

• Implement systems to ensure residents are discussed when necessary and followed up within an agreed timescale.

• Coordinate the identification of high risk patients through utilisation of risk stratification tools such as the WSIC dashboard, PARR 30 etc. and work closely with the multi-disciplinary team to identify a list of patients most at risk of needing extra care.

• Be the main point of contact for Care Homes and the Care Home Support Team and signpost and liaise with multi provider agencies to support the implementation of residents care plans.

• Where care home residents have been admitted to hospital, support the Care Home Matrons where needed to liaise closely with the hospital discharge coordinator to aid the safe discharge and transfer back to their normal place of residence.

• Identify opportunities for service improvement and help to develop plans to implement and monitor improvements.

• Support the wider Care Home Support Team in minute taking of additional meetings where appropriate

• Provide logistical support to any Care Home Support staff delivering training and development projects within the Care Homes

Information Management

• Maintain accurate and contemporaneous records using clinical record keeping system such as Emis web and CMC, ensuring that they remain confidential and secure at all times.

• Actively supports and contributes to the development of key performance indicators for the successful assessment of performance.

• The post holder is responsible for maintaining a working knowledge of current and proposed legislative and statutory guidance in relation to their practice and is responsible for maintaining their CPD. The post holder will identify and agree with their line manager objectives to be achieved throughout the year.
