We are looking for two Care Navigators to join our team at our Long Melford & Lavenham Practices, part of Unity Healthcare.

Its important that you can work well as part of a team as well as independently
You'll need to be able to pick up new IT systems with supportive training

Contract type - Permanent Part Time. Both roles will work across both sites.

Role 1 07:45 - 18:30 Wednesday and Friday - 19.75 hours per week
Role 2 07:45 - 18:30 Monday, Tuesday and Friday - 29.25 hours per week

Main duties of the job

Whats the role?

Youll be the first point of contact for patients accessing our surgery
Youll help ensure our patients can access care at the right time, with the most appropriate clinician
The aim is to reduce the reliance on GP time, reduce inappropriate referrals and avoid unnecessary hospital admissions.
Youll help enhance the care that our patients receive, by handling queries from patients in relation to appointments.
