Job summary

An exciting opportunity has arisen for an enthusiastic and experienced Senior Clinician( RGN,RMN, RNLD ) to join the Continuing Health Care Team, within East Riding CCG.This is an integrated team compromising of Clinicians / Social Workers / COP Lawyers, supported by a dedicated Admin Team.In this role you will support the Operational Manager in coordinating the application of the National Framework in assessing peoples eligibility for funded NHS care, working alongside other Senior Clinicians / Senior Social Worker to undertake and support the decision making where there are complex, high risk issues.

Main duties of the job

As a Case Manager you will promote best practice, leadership and be responsible for providing expert professional advice and guidance to colleagues, promoting excellent working relationships, liaising with colleagues from other disciplines, both externally and internal to the CCGThe post holder will be expected to take a lead in supporting professional development within the team and supervise the Band 6 Clinician, identifying their developmental needs in line with the appraisal process.

About us

The Clinical Commissioning Group lies within the boundaries of the East Riding of Yorkshire covering 1000 square miles of rural, urban and coastal geography and is responsible for the commissioning of healthcare services for the majority, but not all, of the East Riding of Yorkshire population (the Pocklington population are the responsibility of the Vale of York CCG). As such we work closely with our neighbouring CCGs to promote consistency of commissioning with our major provider organisations and with East Riding of Yorkshire Council to ensure that we are jointly commissioning and transforming services to improve the overall health and wellbeing of our population.The CCG consists of 36 GP Practices who have come together to jointly commission services for their patient populations.

Manage a caseload of complex cases and chair complex case conferences where appropriate

Undertake and support the decision making where there are complex, high risk situations this will include supporting social workers and clinicians with legislative dilemmas and complex mental capacity/best interest decisions

Give guidance and expert advice on a daily basis to qualified staff. Model good practice in the team and take a lead in supporting the professional development of the team workforce. Supervise clinicians and qualified social work staff, identifying their development needs in line with the appraisal process

Within delegated authority agree a range of support to meet the identified outcomes of eligible adults

To promote and model the use of critically reflective practice

To implement a joint training and development program to be delivered to health and social care staff working in hospitals and the community

Support the development and implementation of Continuing Healthcare and Funded Nursing Care policies and processes in order to adhere to National Framework guidance and legislation.

Work closely with the Local Authority to negotiate contracts ensuring equity, quality, value, and appropriate services are commissioned for patients. Work with the Local Authority to jointly review and monitor care home placements and complex care packages.

Provide cover for annual leave, sickness and planned/unplanned absences as agreed and deputise for the CHC Operational Manager, as necessary.

To undertake a range of other duties which reasonably corresponds to the general character of the post commensurate with its level of responsibility

Essential

  • Detailed knowledge of National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care
  • Experience of contribution to the development of training programmes/courses and delivering Training.
  • Up to date knowledge of Government guidance and policy developments in primary and social care including partnership working, NSFs
  • Registered General Nurse, Mental Health Registered Nurse qualification or Learning Disability Nurse qualification
  • ENB 998 or equivalent teaching and necessary certificate
  • Evidence of post basic training and/or significant experience in holistic assessment, which may have been gained in the public, private, independent or voluntary sector.
