Job summary

As a Complex Clinical Case Manager the post holder will work as part of a dynamic team in delivery and transformation of care for people with mental health or learning disabilities.

Working autonomously and taking accountability for their own professional actions the post holder will commission and case manager the care of vulnerable people specifically those with mental illness and/or a learning- disabilities, Acquired Brain Injury and those on the Autistic Spectrum Disorder.

Main duties of the job

Working in partnership with health and social care professionals to ensure bespoke commissioned packages of care provide safe clinical practice which delivers positive patient outcomes and value for money for vulnerable people.

The post holder will have case management responsibility for service users in the community within the East Riding or placed elsewhere in the region who originate from the East Riding

The successful applicant will be able to demonstrate high levels of skill and knowledge working with complex individuals, their families and carers within a variety of community care settings. A contemporary understanding of current policy and evidence-based approaches as well as a robust understanding of approaches to managing complex populations in community settings. A good working knowledge of the Mental Health Act (1983), and the Mental Capacity Act (2005) will be vital.

About us

The Clinical Commissioning Group lies within the boundaries of the East Riding of Yorkshire covering 1000 square miles of rural, urban and coastal geography and is responsible for the commissioning of healthcare services for the majority, but not all, of the East Riding of Yorkshire population (the Pocklington population are the responsibility of the Vale of York CCG). Case manage patients with highly complex histories who are a risk of exhibiting extreme and unpredictable levels of challenging behaviours. This will involve providing highly specialist advice relating to their individual care package and planning for future care provision.

Working in partnership with East Riding of Yorkshire Council adult and children directorates – review bespoke packages of care for people with diagnosed mental health and/or learning disabilities who reside within the Community. The post holder will work closely with East Riding of Yorkshire Council colleagues to ensure there is a co-ordinated approach to commissioning and case management of care which results in robust and effective care packages.

To support and when required drive forward the process for ensuring smooth and safe transition of care packages ensuring all agencies are involved and the individual is at the heart of their care pathway.

The post holder will provide relevant written statistical reports and presentations to NHS, Local Authority and Criminal Justice system which inform, educate and provide clinical expertise to support and enhance partnership working.

Manage complex, sensitive and contentious patient information during interactions with legal representatives, criminal justice and healthcare provider clinical teams. The post holder will need to use a high level of communication skill to ensure messages are understood where there may be barriers to understanding and gain agreement and co-operation.

Meet with patients and carers who receive bespoke packages of care to ensure clear understanding of case management ensuring the individual patient is fully consulted and involved in their care pathway.

Represent CCG at patient Care Programme Approach (CPA) review, Care (Education) and Treatment Reviews (C€TRs), Section 117, Mental Health Review Tribunals (MHRT), pre-discharge meetings to ascertain appropriateness of placements

Request patient review meetings when concerns are raised by patient, their carer or by CCGs to discuss quality of care issues and figure care pathways.

Working with East Riding of Yorkshire Council identifies alternative service providers which offer effective safe service provision.

Provide recommendation to the NHS East Riding of Yorkshire CCG Mental Health Funding Panel – operating within the parameters of the NHS East Riding of Yorkshire CCG Mental Health Funding panel policy and Out of Area Policy.

Partnership and Cross Boundary working

Work collaboratively with care providers and care partners to develop positive working relationships.

When required provide advice, guidance and training in own specialism to non-clinical staff within the CCG and East Riding of Yorkshire Council.

To work across multiple care providers – health and social care

To work with senior officers within health, social care and criminal justice

To work with other case managers in the management of cases that have care pathway links to the local area, specifically complex or disrupted care pathways

To work in collaboration with the criminal justice system including MAPPA – Level 2 and Level 3 - providing expertise and advice on the interface of individuals who are in receipt of custodial care and who have complex healthcare needs with associated risks to themselves or the public. This is often in relation to people who have committed a criminal offence and who are of high interest to the media and public.
