Job summary
The post holder will contribute to the implementation and delivery of the National Framework for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) and be able to apply it within the context of assessment, review and identifying outcomes for the assessed needs of individuals, which will include assessing individuals with mental health needs and/or dementia.

Main duties of the job
o Undertaking robust DST assessments.o Completing timely CHC and FT reviews.o Undertaking duty work, providing specialist nursing advice and support to patients with complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence-based practice.o Plan, implement and evaluate care plans for patients and their carers using evidence based and person-centred principles.o Case Management and case management reviews.o Working both autonomously and collaboratively.

Liasing with families, providers and local authority partners using a high level of interpersonal and communication skills. Undertaking End of Life care reviews, following national guidance for individuals Fast Tracked through the CHC system. Exercising professional judgement as necessary when communicating information on behalf of the CHC service. Providing specialist clinical advice to secondary care, primary care, nursing homes, domiciliary care providers to facilitate the CHC process. Interpreting national and regional guidance relating to the implementation of the National Framework for Continuing Healthcare. Working with relevant health professionals, CHC colleagues, business support staff, senior leads, and other support staff within the CHC team to ensure delivery of key outcomes of the service. Leading the delivery of CHC education programs in line with the latest research, evidence and DH and/or NICE guidance, for example, with health staff in the community, hospital, hospice, nursing homes, GPs and Local Authority staff. Promoting individualised personal care planning and the use of personal health budgets (PHBs) where appropriate.
