Job summary

We are currently recruiting for Complex Care Practitioner to join our Barnet team.

The Complex Care Practitioner will act as an expert providing specialist clinical and care management advice and support for adults who have complex needs living in, or the commissioning responsibility of NCLICB. They will be responsible for undertaking continuing care assessments, reviews and associated activities e.g. Duty desk, LRM's and retrospective claims.

You will ensure all activity is delivered in line with the National Framework for NHS Continuing Healthcare & NHS funded-nursing care (2018).They will support the coordination of training & development and recruitment activity across the service.

Please note that you may be required to travel and work at any establishment at any time, within the location of the North Central London ICB.

Main duties of the job

  • Case Management and case management reviews
  • Undertaking robust DST assessments
  • Completing CHC Reviews
  • Working collaboratively with the team and doing duty
  • Plan, implement and evaluate care plans for patients and their carers using evidence based and person-centred principles.
  • Provide specialist nursing advice and support to patients with complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence based practice.

Detailed job description

  • Undertake CHC activities as described above, in line with the national framework
  • Ensure case reviews are undertaken, monitoring any changes in eligibility as well as ensuring packages/placements are meeting patient's needs appropriately.
  • Ensure services are seamless, user / carer centred and take account of individuals health and social care needs, undertaking risk assessment and management of care needs.
  • Manage and oversee complex cases ensuring needs are assessed and met. Any quality and safeguarding concerns are raised and followed through.
  • To ensure continued delivery (in conjunction with Social Services colleagues) of CHC training to ensure all staff involved with CHC are trained to effectively undertake the CHC process.
  • To assist in managing complaints including face-to-face meetings with service users and families in complex situations which require sensitivity, employing sound communication skill
