CHC Nurse Assessor Neurorehabilitation

Job summary

The post holder will lead on the neurorehabilitation caseload; receiving referrals, liaising with GM and local teams and families, monitoring and reviewing patients to ensure patients are placed in the best cost-effective placements to meet their on-going rehabilitation needs, including stepping down patients to a long-term placement or home after the period of rehabilitation comes to an end.

The post holder will take a lead role in the identification of suitable care providers and will be responsible for ongoing review and reassessment of patients and commissioned packages of care. This will be with a view to ensuring that the package of care continues to meet assessed needs and that patients are progressing in meeting their identified rehabilitation goals.

The post holder will have experience in working in a community setting and working with specialist providers; will have had experience in working in Continuing Health Care / Complex Care and with individualized packages of care. The post holder will need an expert understanding of the Mental Capacity Act and experience in working within the relevant legal frameworks.

The post holder will be expected to work with a wide range of NHS and non-NHS health providers, including General Practice, and with the Local, Authority across the borough.

Main duties of the job

Job Purpose

Bury CCG operate a Continuing Healthcare/Complex Case service on behalf of the registered population of Bury. The post holder will be responsible for co-ordinating the process of determination of eligibility for NHS funding for people requiring neurorehabilitation for patients registered with Bury CCG.

The post holder will lead on the neurorehabilitation caseload; receiving referrals, liaising with GM and local teams and families, monitoring and reviewing patients to ensure patients are placed in the best cost-effective placements to meet their on-going rehabilitation needs, including stepping down patients to a long-term placement or home after the period of rehabilitation comes to an end.

The post holder will monitor the budget associated with the neurorehabilitation caseload and provide a quarterly report on the caseload and budget.

The post holder will be part of the CCG Nursing and Quality Improvement Directorate which is led by the Director of Nursing Catherine Jackson.

There are three departments within this directorate, CHC/Complex Care, Safeguarding and Quality Improvement. This post sits in the CHC/Complex Care team which is led by Eve Moroney.

NHS Continuing Healthcare

NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. In order to receive NHS CHC funding individuals have to be assessed by Clinical Commissioning Groups (CCGs) according to a legally prescribed decision making process to determine whether the individual has a primary health need.

The CHC/Complex Care team manage patient who require on-going fully funded NHS care to meet their needs and are a team of approx 15 expereince nurses and administrative staff. The team has been establish for nearly 3 years as is a positive and dynamic team within the CCG.
