Job summary

The NHS Vale of York CCG seeks expressions of interest for two enthusiastic and motivated individuals to join the NHS Vale of York CCG (soon to be part of the Humber and North Yorkshire Integrated Care System) Quality & Nursing Team as ‘Project Nurses / Allied Health Professionals’. We are seeking individuals with proven experience and knowledge in working with Independent Care Providers and quality improvement.

The post holders will support quality improvement programmes across Independent Care Providers across the geography of the NHS Vale of York CCG. Primary focus will be on supporting implementation of the Enhanced Health in Care Homes Framework and support to Independent Care Providers across the system. The posts will also support carers in the prevention, early identification and management of health conditions for clients. There will be engagement with the wider public to raise awareness and communicate strategies for the promotion of wellbeing.

The post holders will be either a registered nurse or allied health professional with experience of planning, assessment and care delivery in care settings and are passionate about promoting quality of care across health and social care demonstrating the appropriate skill set. The post holders will also have an interest and experience in Quality Improvement and a willingness to undertake training where appropriate.

Main duties of the job

The post holders will be responsible for supporting the implementation of projects, delivering training to Independent Care Providers, collating data, monitoring impact and providing ongoing support to the projects. The post holders will provide advice, support and education to clients and their carer’s. They will act as a client’s advocate maintaining privacy and dignity, and respecting personal choices. The post holders will ensure cost effective use of resources.

The post holders base will likely be at West Offices in York and be part of the Quality and Nursing Team, managed by the Head of Quality Improvement/ Principal Nurse. For the North Yorkshire part of the patch, the post holders will be part of an integrated model with North Yorkshire County Council (NYCC) to bring together the Quality and Nursing Team of the NHS and the Quality Assurance and Improvement Team of NYCC in close collaboration with North Yorkshire CCG. This exciting development allows us an opportunity to work as one team to pilot a fully integrated model to bring together the combined skills of individuals from two organisations to maximise the opportunity to support providers and protect vulnerable residents.

As a Project Nurse/ Allied health Professional, excellent communication skills are required to ensure effective engagement and that information is disseminated with clarity, accuracy and in a manner that may be understood by non-clinical individuals.

Core functions:

To support the implementation of Quality Improvement programmes within care homes and domiciliary care within the Vale of York & North Yorkshire CCGs

To provide accurate evidence based education and specialist support to care staff regarding provision of care and quality improvement often in complex care settings.

To work with Independent care providers managers and staff to understand the importance of the EHCH framework and safety culture using quality improvement tools to facilitate change.

The post holder will be expected to lead on the regular training of staff and contribute towards developing and implementing the Quality & Nursing Team priorities

Plan, design, implement surveys or audits, to assist in the development and improvement of care delivery including evaluation of the QI interventions

To support the Senior Quality Lead to ensure effective and efficient delivery of the Quality Improvement programmes

To provide specialist advice and support the delivery of high quality holistic, evidence based education to Care Homes and Domiciliary Care agencies that is responsive, effective and efficient

To support the Senior Quality Lead in the implementation of Quality Improvement across the Vale of York & North Yorkshire CCGs

To liaise effectively with all other health care professionals involved in the residents/ clients care where required

To work flexibly and innovatively to meet the needs of the care staff and residents/ clients in the delivery of the Quality & Nursing team priorities as required.

Contribute to the local and national agenda to supporting individuals to be cared for in their own home as long as appropriate

Maintain accurate and contemporaneous documentation to ensure timely, effective communication and data collection.

Be personally accountable for maintaining skills, knowledge and competence in the use of systems and databases. The post holder is expected to possess good IT skills and be willing to undergo training as necessary.
