Job summary
We are looking for an experienced project manager to
explore the viability &
feasibility of delivering an integrated social care approach to community
stroke teams across West Yorkshire & Harrogate (WYH) & Humber &
North Yorkshire (HNY) Integrated Stroke Delivery Network ISDN areas.
A gap in the
provision of adult social care (ASC) has been identified across both these
regions in multiple areas. Evidence
demonstrates this gap has a significant detrimental impact on patient and carer
experience, ability to support independence and autonomy, therapy team
capacity, and hospital bed capacity/patient flow.
We are looking
for a project manager with experience of the ASC/rehabilitation interface.
Someone who favours a strengths-based approach and can use co-production to
assess and map the issues affecting ASC integration in stroke rehabilitation.
Ultimately someone who can develop a project which articulates the problem and barriers to achieving fully
integrated social care support.
Main duties of the job
To develop a project
plan for assessing and mapping the potential for optimal integration of ASC
in Community stroke rehabilitation.
To
assess current barriers and factors affecting to ASC integration in key areas
including community care bed, accessing timely care
packages, specialist social worker and
reablement care staff training and education,
To
assess and map the impact of the lack of ASC integration on delayed hospital discharge, Therapy time spent managing social support / care
packages
To map community assets for each area to
enable MDTs to link people in with local organisations and support access to
life after stroke support
To explore opportunities to work innovatively
to improve ASC integration by developing personalised care budgets,
integrated within health and social care budgets, to support more
person-centred care and autonomy.
To
work autonomously engaging with key stakeholders from across the health and
care system to support coproduction in
identifying key initiatives to pilot
To develop a
business plan that outlines where ASC integration can be implemented and
optimised to enable
person-centred and person-led rehabilitation to promote peoples independence
and autonomy in their own home and creating capacity in inpatient and community
rehab services.

To set up a group of
people with lived experience of Stroke, Carers and Peer Leaders to coproduce,
steer and inform key outputs.
To embed a strength
based approach, ensuring that the persons own strengths and preferences are
key to their discharge and community support.
To map current provision
of community care bed options across both ISDNs
To explore the
barriers to patients accessing timely care packages
To develop a system
wide training needs analysis of health and social staff working across the
Stroke Pathway,
To identify the
current situation in terms of number of delayed discharges across pilot sites
within each ISDN
To quantify risk as a
result of non-specialist care package planning e.g. using incident reporting
data and safeguarding cases
To explore rehab time
spent managing social support / care packages
To map community assets for each area to enable
MDTs to link people in with local organisations and support access to life
after stroke support
To explore the potential of developing personalised
care budgets, integrated within health and social care budgets, to support more
person-centred care
To work closely with
the ISDN Health Inequalities lead to prioritise the focus for the delivery of different models in different areas according
to need.
To explore the
barriers and facilitators to develop and sustain an integrated social care
approach into community stroke services identifying the elements which would have
the greatest impact without significant investment
From scoping to identify
key initiatives to pilot in 1 provider and local authority in each ISDN
footprint
To work
autonomously in accordance with relevant guidance, policies, legislation and strategies
to support the delivery of improvements in service performance and national and
local target delivery.

- Educated to Masters level or equivalent experience
- Should have an appreciation of the relationship between NHSE, provider and commissioning organisations and the broader health and care system
- Evidence of continuing professional development
Desirable
