East Riding Clinical Commissioning

The Clinical Commissioning Group lies
within the boundaries of the East Riding of Yorkshire covering 1000 square
miles of rural, urban and coastal geography and is responsible for the
commissioning of healthcare services for the majority, but not all, of the East
Riding of Yorkshire population (the Pocklington population are the
responsibility of the Vale of York CCG). As such we work closely with our
neighbouring CCGs to promote consistency of commissioning with our major
provider organisations and with East Riding of Yorkshire Council to ensure that
we are jointly commissioning and transforming services to improve the overall
health and wellbeing of our population.
The CCG consists of 36 GP Practices
who have come together to jointly commission services for their patient
populations.
We aim to use Clinical Commissioning
to drive the transformation of local services to better meet the needs of our
resident population. This will be achieved whilst continuing to work within the
national operational guidance which underpin the wider aspirations, and
delivery, of the NHS.

We are looking to recruit a dynamic
and innovative Band 6 Continuing Health Care (CHC) Clinician to work as part of
the Continuing Health Care team delivering an effective assessment, reviewing
and case management service across the East Riding for persons who may be
eligible for NHS funding.
The successful candidate will be
joining an integrated team of Nurses and Social Workers
As a Continuing Health Care Nurse
Assessor / Case Manager you will have a defined caseload and be responsible for
assessing, reviewing and the ongoing case management of care packages in line
with National policy. CHC monitors ongoing care needs of patients, ensuring
referral to other agencies as appropriate
You will work closely with the Local
Authority, Acute Trusts, The Independent Sector, Care Providers, Families and
Carers

This role will involve an experienced
clinician who is able to work with patients and families to support the assessment
and review process for NHS CHC and NHS Funded Nursing Care (FNC) in line with
the DoH National Framework for CHC, incorporating the National Decision Support
Tool (DST)
You will work with families and our
Local Authority partners and the wider MDT to undertake comprehensive
assessments and reviews so that eligibility for CHC and FNC can be determined,
providing comprehensive clinical advice and support.
The position will also include
supporting the wider CHC work within the team and you will work closely with
the other CHC Nurses, Social Workers, Legal and Administrative teams to support
these processes
Our overall aims are to ensure
innovative, responsive and effective clinical commissioning and that our
population has access to the highest quality healthcare, providing the best
within available resources. We achieve this by working in partnership with our
colleagues across the health and social care sector.
To follow process set out by the DoH
on the National Framework for CHC & FNC and to provide comprehensive
clinical advice, assessment, case management, review and follow up on all CHC
& FNC functions for the East Riding CCG
To provide support and advice to
other workers, MDTs, carers and patients as required in implementing the
National Framework for CHC &FNC, contributing to the ongoing training of
other nursing colleagues as necessary.
To procure safe and appropriate care
packages based on individual needs.
To assist the Local Authority with
jointly funded packages.
To undertake complex assessments to
identify eligibility for Continuing Health Care and Funded Nursing Care by
completing the National Decision Support Tool. Gather evidence to support complex
clinical judgements, which takes into account the knowledge and advice of other
professionals and relatives views.
To assist with complaints, IRPs and
retrospective cases from a clinical view point.
To conduct ongoing reviews.
To ensure you observe risk, are aware
of the local governance arrangements and work within this when carrying out
duties on behalf of the CHC & FNC department.
To keep accurate, high quality
documentation, and ensure information flow to the business and admin team is up
to date in order that they can manage data and process payments.
To travel where necessary to assess
and review patients.
To carry out any other duties as
needed by the department of behalf of the East Riding CCG.
Infection Control is integral to this
post. The post holder should at all times demonstrate effective infection
control and hygiene practice.
