services. The development of a new integrated service
model across health and social care is a key priority for North Yorkshire place.

work across boundaries and improve the health of their population.
This vision includes
preventative projects and services to address long term conditions and frailty
and help people age well, services to avoid admissions to hospital, integrated
processes with the Local Authority to manage discharge and flow back out to the
community and building capacity to support recovery after a crisis or hospital
episode.
As a service improvement
manager, you will play a leading role in making this exciting vision become a
reality. Working as part of a place-based team, you will design and implement solutions
to strengthen and transform key local services. You will work closely with
colleagues in health and social care, including community provider
organisations across North Yorkshire, North Yorkshire Council, and the voluntary
sector.
Main duties of the job
You will have the skills and
enthusiasm to engage with provider organisations and North Yorkshire Council to
build integrated models and services reflecting the needs of different
communities in North Yorkshire.
You will lead key projects and
develop proposals and business cases to address key priorities, working with
colleagues in other organisations. You will then oversee implementation and
delivery to confirm successful achievement of KPIs and the impact and benefits
to people receiving the services.
You will need to work
flexibly, think creatively, and demonstrate a highly professional business and
person-centred approach to service development.
The post holder will be able
to work on their own initiative, meet competing deadlines and targets and
manage relationships between partner organisations.

Yorkshire Health and Care Partnership (HNY HCP) It covers a population of 550K
and makes up a third of the total population of the HNY HCP.

The post holder will be
responsible for providing commissioning support and project delivery to support
the NY PLACE to achieve transformational change and quality improvement through
the implementation of significant integration and service redesign projects.
These will address key system priorities that contribute towards developing an
integrated health and social care model through engaging with partners across
the NHS, LA and VSCE sectors. Initiatives and projects will deliver the clear
ambition from all partners to develop community services that work across

needs of the local population and is based on sound population health management
data.
Leading specific improvement projects focused
on improving quality.
Responding to the data monitoring of the NY
PLACE with regards quality (core dashboard) and take action accordingly.
Providing a high-level focus on delivery,
working within agreed protocols, underpinned by best practice and with
recognised tools and methodologies.
Providing high quality support that is timely, responsive,
and fit for purpose.
Providing
expertise as required on services in steady state, transitioned and those
ready to be commissioned; including activities to inform a commissioning
decision.

Essential
- Specialist knowledge, training and experience to Masters Level or equivalent.
- Evidence of post qualifying and continuing professional development.
- Must have an understanding of the background to and aims of current healthcare policy.
- Should have an appreciation of the relationship between the Department of Health, NHS England, the Local Area Team, NY PLACEs and individual provider and commissioning organisations
Desirable
- Project Management qualification. e.g. Prince2 and managing successful projects
Experience
Essential
- Experience of identifying and interpreting National policy. Experience of researching best practice (globally, private and public sector), interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform (advising on policy implementation.
- Experience of developing and delivering service improvement
- Working knowledge of Microsoft Office with intermediate keyboard skills.
- Experience of using data bases and analysing performance reports.
- Experience of the commissioning process gained at a senior level within an NHS organisation, or equivalent.
- Experience of project management methodology

- Experience of using capacity and demand modelling techniques to inform planning decisions.
- Experience of service development in a community or social care-based setting
