This job is now closed

Job summary

This post is a rewarding and exciting opportunity to improve the quality and outcomes of community-based services. The development of a new integrated service model across health and social care is a key priority for North Yorkshire place. There is a clear ambition from all partners to develop community services that work across boundaries and improve the health of their population.

This vision includes preventative projects and services to address long term conditions and frailty and help people age well, services to avoid admissions to hospital, integrated processes with the Local Authority to manage discharge and flow back out to the community and building capacity to support recovery after a crisis or hospital episode.

As a service improvement manager, you will play a leading role in making this exciting vision become a reality. Working as part of a place-based team, you will design and implement solutions to strengthen and transform key local services. You will work closely with colleagues in health and social care, including community provider organisations across North Yorkshire, North Yorkshire Council, and the voluntary sector.

Main duties of the job

You will have the skills and enthusiasm to engage with provider organisations and North Yorkshire Council to build integrated models and services reflecting the needs of different communities in North Yorkshire.

You will lead key projects and develop proposals and business cases to address key priorities, working with colleagues in other organisations. You will then oversee implementation and delivery to confirm successful achievement of KPIs and the impact and benefits to people receiving the services.

You will need to work flexibly, think creatively, and demonstrate a highly professional business and person-centred approach to service development.

The post holder will be able to work on their own initiative, meet competing deadlines and targets and manage relationships between partner organisations.
