This post is a rewarding and

services. The development of a new integrated service
model across health and social care is a key priority for North Yorkshire place.

ambition to radically reshape the delivery of intermediate care and develop
schemes and services through the Better Care Fund. This includes building
capacity to prevent avoidable admissions through preventative work and crisis
response services, combined with delivering a Home First ethos following
discharge from hospital to improve longer term outcomes.
As a senior service
improvement manager, you will play a leading role in turning this exciting
vision become a reality. With the support of a place-based team, you will take
forward the transformation of key local services as part of the North Yorkshire
integrated health and social care model. You will forge strong partnerships
with community provider organisations across North Yorkshire, North Yorkshire
Council and the voluntary sector, helping the system to work together to
deliver shared goals.
Main duties of the job
You will have the experience
and capabilities to undertake service transformation within a complicated
provider landscape. You will engage with different provider organisations,
stakeholders and local communities to build integrated services that meet the
differing needs of communities in North Yorkshire.
You will lead key programmes
of community-based development, including utilising opportunities for integration
through the Better Care Fund. You will work
with colleagues in other organisations to plan and develop transformational
initiatives. You will ensure the planned benefits are successfully delivered,
including successful achievement of KPIs and the impact and benefits to people
receiving the services.
You will work with multiple
system providers to improve the flow of patients (front door to back door)
through delivery of a range of inter-related projects concerned with urgent
care, flow and discharge. You will also support a broader programme of
engagement with community service partners to deliver proactive care for people
with frailty to reduce crises and hospital admissions.

flexibly, think creatively, and demonstrate a highly professional business and
person-centred approach to service development.

Yorkshire Health and Care Partnership (HNY HCP) It covers a population of 550K
and makes up a third of the total population of the HNY HCP.

Yorkshire Place has a complex geographic and partnership landscape with
multiple providers including 5 acute trusts, 4 NHS community providers, 14
Primary Care Networks and 65 general medical practices.

Essential
- Specialist knowledge, training and experience to Masters Level or equivalent.
Desirable
- Prince 2 or programme management qualification or relevant equivalent experience

- Significant NHS management experience in an Operational Role in either a provider or commissioning organisation
- NHS management experience in a corporate planning/transformation role in either a provider or commissioning organisation
- Evidence of leading and delivering change
- Service planning and redesign experience
- An understanding of the NHS financial regime and current healthcare policy
- Experience of researching best practice, interpreting its relevance and processes/ practices which could be implemented successfully to achieve system reform (advising on policy implementation)
- Experience of managing and motivating a team and reviewing performance of the individuals.
- Experience of identifying and interpreting national policy.
- Demonstrable experience of dealing with a wide range of people at all levels and with both Clinicians and Managers
