This is a great opportunity
for an experienced manager to both assist and provide leadership in the
transformation of stroke services across Humber and North Yorkshire playing a
key role within the Humber and North Yorkshire Integrated Stroke Delivery
Network.
Integrated
Stroke Delivery Networks (ISDNs) are the key vehicle for transforming stroke
care across the country. ISDNs work with partners from across their regions to
help prevent stroke, improve diagnosis and improve access to treatment in 24/7
specialist stroke units.
The successful
post holder will work alongside designated clinical leads, along the length of
the stroke treatment and care pathway from responding to suspected onset of a
stroke, though to diagnosis, treatment and rehabilitation.
The role will also
include leading stroke prevention initiatives (working in collaboration with
Humber and North Yorkshire Cardiovascular Disease Prevention Network).
There is both a
mature work programme of community based initiatives which target the quality
of life of individuals after a stroke, including access to rehabilitation,
support (both for individuals who have had a stroke and the staff whose role(s)
provide support to them).
A number of
these projects are collaborative arrangements with neighbouring ISDNs, which
presents an opportunity to develop a regional perspective of service provision.
Main duties of the job
In addition to
working with and alongside statutory NHS/ Local Authority services, Humber and
North Yorkshire ISDN also has a positive and proactive working partnership with
third sector organisations such as the Stroke Association and has newly
developed collaboration with individuals who have had a stroke and/or their
carers.
Projects will
also include working with ambulance services and receiving hospitals regarding
access to hyper acute stroke centres and the imaging and treatment pathways
within these centres and if appropriate, referral onto specialist centres
which provide mechanical thrombectomy services.
Proactive
support for the post holder will be provided by other team members which
include four clinical leads.
The key focus of this role is
to:
Support
the development and implementation of the HCV ISDN Portfolio and to lead on
specific programmes and projects as required.
Support
the development and implementation of the HCV ISDN programme engaging with
stakeholders across the health and care system.
Support
collaboration and innovation in stroke care
To
work closely with other stakeholders across the health and care system to
design and deliver our stroke ambitions.
The
postholder will need to be flexible in their approach to work, and able to move
easily between different programmes of work at different levels of detail being
able to work with information at service details through to strategic plans/
planning.
