Job description

Job responsibilities

The Inclusion Health Programme manager will be responsible for the development, implementation, and on-going delivery of the inclusion health work-plan for Humber and North Yorkshire.

This is an exciting and rewarding opportunity that will allow the successful applicant to oversee a vital and urgent agenda for improving the health and wellbeing of some of the regions most vulnerable and marginalised groups. This will require a holistic and collaborative approach that recognises the complex and interrelated factors that affect the health of inclusion health groups and the barriers they face in accessing health care and will involve taking a trauma informed approach to working with people, communities, organisations and systems and being a strong advocate for this approach.

The Programme manager will be responsible for the establishment and coordination of partnership efforts to understand the causes of ill health and wellbeing among inclusion health groups, and identify the greatest opportunities to work together to tackle these issues.

This approach will aim to improve the physical and mental health outcomes of inclusion health groups across Humber and North Yorkshire. It will include action to reduce the occurrence of ill-health and improve wellbeing, including addressing wider determinants of health. This will rely on partnership working with communities, organisations, local authorities, people with Lived Experience and individuals with a relevant role or interest in this across the Humber and North Yorkshire area.

Inclusion health groups are a relatively small but significant population, with high health and social care needs, who face a range of barriers in accessing services. Inclusion health is a catch-all term used to describe people who are socially excluded, typically experience multiple overlapping risk factors for poor health (such as poverty, violence, and complex trauma), experience stigma and discrimination, and are not consistently accounted for in electronic records (such as healthcare databases). These experiences contribute considerably to increasing health inequalities and frequently lead to barriers in access to healthcare and extremely poor health outcomes, which evidence tells us are overall worse than the general population.

Work will target although not be limited to the following populations:

People who experience homelessness

People with drug and alcohol dependence

Vulnerable migrants and refugees

Gypsy and traveller communities

Roma Communities

People in contact with the justice system

Sex workers

Other marginalised groups e.g., victims of modern slavery
