Yorkshire.
As
our Programme Manager, you will lead a vital initiative aimed at improving the
health and well-being of communities that are exposed to extreme social
exclusion and health inequalities. This role is an opportunity to create
lasting impact at the forefront of change, working collaboratively to break
down barriers and improve access to healthcare for inclusion health groups.
This is an exciting, varied,
and challenging role working within the Chief Operating Office for our growing
Population Health and Prevention Programme. As the Humber and North Yorkshire
Inclusion Health Programme Manager you will be responsible for the
establishment and coordination of partnership efforts to understand the causes
of ill health and wellbeing among inclusion health groups and identify the
greatest opportunities to work together to tackle these issues.
You will be responsible for identifying and
addressing population health and prevention requirements and setting the
strategic direction for these areas, enabling the ICB to deliver its overall
ambitions to improve life expectancy and reduce the gaps in healthy life expectancy
in inclusion health groups.
Main duties of the job
The Inclusion Health Programme
Manager will be responsible for supporting work to improve physical and mental
health outcomes of inclusion health groups across Humber and North Yorkshire.
It will include action to reduce the occurrence of ill-health and improve
wellbeing, including addressing wider determinants of health. This will rely on
partnership working with communities, organisations, local authorities, people
with Lived Experience and individuals with a relevant role or interest in this
across the Humber and North Yorkshire area.
The
postholder will oversee the development of a Humber & North Yorkshire inclusion
health agenda by applying a trauma informed approach across the ICS. They will
lead on the priorities of the Population Health and Prevention Executive Committee
in conjunction with the ICB, places, and providers, and will be responsible for
identifying and addressing population and prevention requirements.
You
will be able to communicate highly complex information relating to the
monitoring and delivery of the programmes which may prove contentious due to
the potential financial and activity impacts arising from the analysis of the
available information.
The
post holder will be responsible for managing and attending meetings of the
Population Health & Prevention Programme Board and Population Health &
Prevention Committee alongside the Senior Clinical Lead and Consultant in
Public Health.

will require a holistic and collaborative approach that recognises the complex

systems and being a strong advocate for this approach.

not consistently accounted for in electronic records (such as healthcare
databases). These experiences contribute considerably to increasing health
inequalities and frequently lead to barriers in access to healthcare and
extremely poor health outcomes, which evidence tells us are overall worse than
the general population.

- Degree in Public Health or related field e.g., Health Promotion, or evidence of equivalent learning in public health
