Job summary

This newly created role provides an exciting opportunity to work across Humber and North Yorkshire supporting the diabetes, respiratory and stroke (ISDN) clinical networks.

Working with clinical and programme leads you will plan, coordinate and lead on the development and management of the programmes transforming services to improve patient outcomes and experience and to support the delivery of system priorities.

The role will be instrumental in developing joint working across the system with a range of stakeholders to address population health priorities, reduce health inequalities and support service needs as well as providing assurance on the delivery of all aspects of the agreed work programmes and the management and mitigation of associated risks and issues.

Main duties of the job

To plan, coordinate and lead on the development and management of agreed programmes, specifically Stroke, Respiratory and Diabetes to transform services to improve patient outcomes and experience and to support the delivery of system priorities.

To provide assurance on the delivery of all aspects of the agreed work programme and the management and mitigation of associated risks and issues.

To develop and promote joint working; working with system stakeholders across health and care including primary care, community and secondary care providers, local authorities and VCSE organisations to address population health priorities, reduce health inequalities and to support service needs in Humber and North Yorkshire.

To provide support, as appropriate to other programmes/projects managed by project managers from other ICB teams or partner/provider organisations. Provide guidance and advice whilst building skills and capabilities in the team.

To support the ICB approach in the development of systemwide plans in the context of local and national priorities and to help inform the ICBs strategic direction in respect of management of long term conditions

To develop, lead on and be responsible for the delivery of a range of complex specific programmes and projects across the ICB to support the health and care system in delivering their plans

To highlight risks and barriers to delivery working with providers and partners to provide a solution based approach

Using agreed Project Management Office (PMO) tools, produce business cases, service specifications and financial plans to support the delivery of programme plans

To proactively engage with regional and national teams to ensure that the ICS is aware of any new national and local guidance or policy changes that will impact on service delivery or performance.

To identify and be responsible for scoping of outcome improvement and value improvement opportunities

To support the ICS to identify variation and best practice and support improvement and implementation

Develop a working knowledge and understanding of the ICS, its localities, cultures, people, communities and services

To manage performance against an agreed work programme

Work with partners to engage in the development of effective local arrangements for involving patients, members of the public and other partners as required

Ensure that effective evaluation arrangements are established for all projects and that evaluation is carried out in a timely and effective manner

Work closely with the system leads and act as the key link and point of contact to support development and implementation of a system wide strategy and workplan

  • Evidence of post qualifying and continuing professional development.
