Job summary

An exciting opportunity has arisen for an Admission Avoidance Manager for Adults to join a dynamic team in the delivery and transformation of care for people with learning disabilities and / or autism. This post is hosted by East Riding of Yorkshire Health and Care Partnership but supports the Humber Transforming Care Programme across the Transforming Care Programme area; i.e. East Riding of Yorkshire, Hull, North Lincolnshire and North East Lincolnshire. The Admissions Avoidance Manager for Adults works as part of the Transforming Care Programme to chair Care (Education) Treatment Reviews for people with a learning disability and/or autism who are either currently inpatients within mental health or specialist hospitals, or those who are at risk of admission.

The post holder will be required to travel to carry out care and treatment reviews in person, usually once every six months . People with learning disability and/or autism spectrum conditions can sometimes be admitted to hospitals across England or Wales, as well as within the local Humber region however people placed a great distance out of area are in the minority.

Main duties of the job

The Care (Educations) Treatment Review process is very closely linked with the new Humber Dynamic Support Register (DSR) model which has been designed and implemented across the region to facilitate effective multi-agency collaboration for the mitigation of risks such as placement breakdown, hospital admission or contact with the criminal justice system. The post holder will coordinate multi-agency action planning and risk management with health, social care, education and criminal justice professionals, and the individuals themselves to feed into the DSR following reviews to ensure that recommendations and outcomes from a C(E)TR are achieved. This will involve the discussion of highly complex, sensitive and contentious information, where persuasion, empathetic and re-assurance skills are required in order to gain agreement or co-operation.

The role of the Admissions Avoidance Manager for Adults is a vital part of the Transforming Care Programme, and someone with a clear understanding of the delivery of safe and effective care across a wide range of providers is required. We are looking for an individual who is able to take a lead role for ensuring that people are supported to be as independent as they can be and stay safely at home wherever possible. The post holder will be a confident communicator, have expertise in learning disability and autism with either a learning disability nursing or social care equivalent background, and demonstrate flexibility in their approach to each individual and MDT that they support.

The post holder will report to the Clinical Lead (Nurse) Adults Transforming Care, and work across the Humber region to assist colleagues and partner organisations to embed the DSR and C(E)TR processes within local services for adults with learning disability and/or autism spectrum conditions, working within both national and local policy and guidance. As part of the newly established Admissions Avoidance Hub the post holder will support their Admissions Avoidance Manager for Children and Young People colleague, to ensure effective and successful transition from Childrens and Adults services for the individuals known to the DSR, and to deputise for one another to provide service cover when required.

The postholder will provide a chair function for both community and inpatient C(E)TRs providing highly specialised advice for people with learning disabilities and/or autism who display behaviours which challenge under the Transforming Care Partnership programme.

The postholder will respond to requests to chair Local Area Emergency Protocol (LAEP) meetings, ensure that any actions are followed up and use the outcome to inform the Dynamic Support Register (DSR).

The postholder will be occasionally required to provide cover and support for colleagues in the admission avoidance hub, which supports people with a learning disability and autistic people of all ages through implementation of C(E)TRs and dynamic support systems.

The post holder will lead on the interpretation and implementation of the Dynamic support register (DSR) and care (education) and treatment review (C(E)TR) policy, 2023.

The aim of the C(E)TR is to bring a person-centred and individualised approach to ensuring that the care and treatment and differing support needs of the person and their families are met, and that barriers to progress are challenged, overcome and analysed, options compared and reviewed to agree the bestway forward. This analysis can be complex in terms of the nature of the role looking at the patients care and treatment options, legal status, capacity including capacity to care and treatment, residency and finances.

The post holder will coordinate action planning with health, social care, education and criminal justice professionals, and families following reviews to ensure that recommendations and outcomes from a C(E)TR are achieved.
