Humber and North Yorkshire ICB
(place based
at North Lincolnshire) is looking for an experienced mental
health or Learning disability Nurse to join our case management team in a fixed term or secondment capacity for a 12 month period.
As a Clinical Case Manager you will work as part of a
dynamic team in delivery and transformation of care for people with complex
health needs, mental health or learning disabilities.
Working autonomously and taking accountability for your own
professional actions the post holder will commission in line with the ICBs
processes and case manage the care of vulnerable people specifically those with
complex health needs, mental illness and/or a learning disabilities, Acquired
Brain Injury and those on the Autistic Spectrum Disorder.
Working in partnership with North Lincolnshire Council,
adult and children directorates, health and social care professionals to ensure
bespoke commissioned packages of care provide safe clinical practice which
delivers positive patient outcomes and value for money for vulnerable people.
Main duties of the job
Case manage patients who have complex health needs and who may have
highly complex histories who are a risk of exhibiting extreme and unpredictable
levels of challenging behaviours. This will involve providing highly specialist
advice relating to their individual care package and planning for future care
provision.
To support
individuals to access healthcare and support through Personal Health Budgets.
This will include calculating the cost of available options, providing
recommendations and financial analysis to the Placement Provision and Review
Meeting through a personalised care of outcomes and measures.
Working in partnership with North Lincolnshire Council Adult services
review bespoke packages of care for people with diagnosed mental health and/or
learning disabilities who reside within the Community. The post holder will
work closely with North Lincolnshire Council colleagues to ensure there is a
co-ordinated approach to commissioning and case management of care which
results in robust and effective care packages.
To support and when required drive forward the process for ensuring
smooth and safe transition of care packages ensuring all agencies are involved
and the individual is at the heart of their care pathway.

- Working knowledge of IT systems and software programmes such as Outlook, Word and Excel
- Knowledge of Mental Health Act and relevant legislation in relation to Mental Illness and Learning Disabilities.
