Job summary

The Director of Nursing is looking to recruit 3 Primary Care Improvement leads to support the quality improvement within primary care to meet the Integrated Care Board statutory and regulatory requirements for quality and safety for our population across NWL ICB Primary Care services. These posts are part-time working 3-days per week.

Main duties of the job

The post holder will support the Director of Nursing for Primary Care and the Senior Quality Improvement Lead on the quality assurance mechanisms in order to achieving sustained improvements in quality of care, patient safety and patient experience within primary care.

You will be expected to matrix work with colleagues across North West London and other ICBs. The post holder will need to be able to demonstrate excellent organisational skills, attention to detail and be able to build a good working relationship with both internal and external stakeholders.

The interviews will provisionally take place week commencing 7 October 2024

Operational

To support Primary Care Quality leads and the Director of Nursing-Primary Care in the quality and assurance process.

To support the Senior Primary Care Improvement Lead in ensuring robust processes are in place for managing Serious Incidents under the Patient Safety Incident Reporting Framework (PSIRF) and Learning from Patient Safety Events (LFPSE).

To support the Primary Care GP Practices in ensuring the provision of the proper high-quality care to the right patients and service users at the right time by the right staff.

To support the provision of high-quality care for patients and ensure systems are in place to report and learn from the incidents.

To lead specific Primary Care projects such as Cervical Screening and support primary care programs such as access, immunisation and vaccination, GPN modelling workforce transformation, and advanced clinical practitioner maturity, to name a few.

To champion the use of data and evidence to drive local health and inequality improvements

To provide patient safety advice to staff across the Primary Care and liaise with the ICB of Patient Safety specialists within the Quality Team.

To work with the Primary Care Networks (PCNs) and GP Practices to ensure the embodiment of quality improvement principles in reducing service provision variation and improving patient safety and patient experience.

To support having an environment in which efficient delivery of patient care is in line with fundamental and evidenced-based standards to ensure a framework of safe practices exists.

To support the development of a patient safety culture and safety systems and have sufficient seniority to engage directly with the borough-based team.

The post holder is pivotal in providing quality assurance to the ICB and supporting the implementation of the Primary Care Quality Strategy.

To support the Director of Nursing for Primary Care in ensuring the quality of care provision radiates across all four Primary Care Strategy offerings (Access, Proactive Care, Preventative Care and Planned care).

To support improving the local population's health outcomes and standardising serious incident reporting in Primary Care.

To support the development of a culture that encourages openness in reporting and ensures that the lessons learned from adverse incidents are shared within Primary Care.

To work closely with the quality team, Primary Care contracting team, and the Learning hub to have intelligence on care quality information and data to support quality improvement.

To collaborate with the Primary Care and the Imperial Partners on safety matters and escalate to the Director of Nursing for Primary Care on patient safety incidents, complex complaints, Care Quality Commission action plans and any identified risks in the system.

To support the increase in uptake of digital incident reporting solutions within the Primary Care.

To proactively create an open, inclusive, equitable culture. Ensure the contributions and perspectives of all staff are heard and valued and influence decision-making within the team.

To support incorporating quality improvement methodologies in Primary Care Nursing, Allied Healthcare Professionals, Community Nursing, Additional Roles Reimbursement Scheme (ARRS) workforces' transformation work streams, leadership programs and clinical supervision processes.

To support Primary Care Programs such as Cervical Screaming, Access, Immunisation and Vaccination, GPN Modelling workforce transformation, and Advance Clinical Practitioner maturity, to name a few.

To support the NWL Primary Care Reference Group, NWL Clinical Advisory Group, local Clinical Interface Group/Senate, Patient Safety Committee and other internal and external groups/committees as required.
