Clinical leadership and expertise are identified as essential for delivering the North West London Integrated Care System vision and objectives. The clinical leadership works together across the Integrated Care System to focus on service improvement, service redesign and the development of care pathways from primary care through to secondary care and community-based services.

This is an exciting opportunity to join a dynamic and committed Integrated Care System health and care leadership team to shape the impact of primary care leadership in North West London.

We are looking to build a team of primary care clinical leads that reflects our local communities, as well as representing the full range of care delivery expertise from across general practice and primary care network teams. This includes GPs, nursing staff, therapists and allied health professionals, alongside other roles.

The North West London Primary Care Clinical Lead roles have an important role within the Integrated Care System, co-leading on the development, improvement and redesign of care pathways with clinicians from interfacing sectors. The post holder will bring with them current health experience and thinking which drives improvements in care, long-term preventative strategies for population health management and for tackling health inequality.

This role requires strong leadership skills, critical reasoning, skills supporting collaborative working, good governance and delivering impactful public outcomes.

The Urgent and Emergency Care (UEC) Primary Care Clinical Lead will provide expert clinical leadership and guidance, oversight, support and recommendations in relation to North West Londons approach to the provision and delivery of UEC services. This will support the ICB and the ICS to deliver the priorities agreed for NW London, the national UEC recovery programme and the NHS Long Term Plan. Key pathways, workstreams and responsibilities within this include:

The Clinical Leads should ensure that the requirements and outcomes for all ages, including babies, children and young people, adults and older adults is central to the UEC programme. A population health approach should be used to consider the needs of different generations at their different life stages and the transition between these stages e.g. transition from adolescence to adulthood.

Core Functions

provide the primary care perspective for the UEC Programme and related areas

think and act strategically about service development needs

provide NW London system-wide clinical leadership with a focus on improving quality of care and reducing variation in outcomes for our resident population

support development of clinical pathways in line with clinical effectiveness

drive delivery of clinical pathways and improved outcomes in primary care across NW London

work to address inequalities and reduce variation in outcomes across NW London

Key priorities of clinical leadership in NW London are to help drive effective efforts to reduce health inequalities and improve life expectancy; ensure people get access to the right treatment, at the right time, in the right place; and help people to recover and stay well. This is now in the context of increased demand and greater morbidity as a result of Covid-19, meaning that greater importance is placed on:

better use of digital technology to deliver safe, flexible care;

easy access to the right care and support, with prompt response in a crisis/emergency; and

different ways of working and supporting our staff.

UEC Programme

To represent NW London as Clinical Lead at the Regional Primary Care and UEC meetings. To share learning across both programmes and highlight issues for addressing within the ICB where required

Member of the NW London Urgent and Emergency Care Board

Co-chair the emerging UEC Clinical Reference Group (CRG)

Support the development of medium and long term ICB and ICS strategies

Support all UEC Programme officers with the clinical aspects of their workstreams, including clinical leadership and expertise in the development of workplans and strategies

Ensure clinical engagement and input from clinical leads from across healthcare settings, including acute, community, mental health and primary care

Work with relevant Clinical Reference Groups (CRG) to support those CRGs in delivering the UEC programme and related objectives

Chair agreed task-finish groups to deliver against the agreed CRG work programmes

Attend UEC Team meetings to provide clinical leadership and direction.

UEC Pathways

Provide clinical leadership for the development of cross cutting pathways that are delivered across the ICB, providers and health care setting

Support the integration of UEC and Primary Care services by supporting the development of the Same Day Access programme, linking in with officers across the UEC, Primary Care and Borough teams

Support access, demand, surge and recovery initiatives. This will include the whole system transformation of clinical pathways and services across acute, community, mental health, 3rd sector and other relevant settings

Implement of national clinical recommendations to deliver top decile performance and address inequalities of access and outcome

Ensuring that pathway development is provider agnostic and is in the best interest of the patient

Work with Clinical Leads for chronic and long term conditions to develop pathways that span planned and unplanned care settings, maximising the use of proactive care where appropriate

Work with clinical leads for mental health to ensure that patients are treated quickly in the most appropriate setting

Provide primary care leadership, input and expertise across the ICB; which is likely to include:

-acting as the primary care advocate for pathway changes and other service adjustments, whether in hospital or the community

Chair the monthly contract 111 CQRG meetings and review of monthly reports. This includes deep dives into pathway subjects and investigations of all SIs

Attendance at finish / task groups will be required for individual sign off for Clinical Pathways for NW London, National or Regional as a working group. This will include stakeholder Clinicians to resolve and discuss any issues

Represent NW London in Regional Clinical Governance monthly meetings. This will be to review any SIs including patients and providers for the 111 service. Included in this will be reviews of proposed Regional models of care and governance from a NW London perspective

Represent NW London as Clinical Lead in attendance for all Regional IUC Forums. These will include Clinicians from London providers and ICBs. Transformation of services are reviewed with detailed discussions with providers

Act as the Clinical SRO for the ICB for any business continuity process relating to the 111 Service and agree immediate change to pathways if required, for example cyber-attacks or COVID

To agree and sign off COVID pathways for all UEC pathways. This will include addressing activity profiles and coding to agree change where necessary

Meeting with 111 operational Lead to review clinical concerns, National guidance and pathway transformation.

Directory of Services (DoS)

To be the accountable Clinical Lead for the Directory of Services (DoS) for NW London and attend weekly DoS meeting to review and sign off all clinical pathways relating to the DoS/MiDoS for NW London services. This includes all pathways that are profiled on the DOS for all providers in NW London

Rolling programmes are in place for continual review for the DoS pathways including development and design. National Updates will be given for this role to sign off and profiled for implementation into the NW London system

Represent NW London as Clinical Lead at the Regional DoS Board on a monthly basis. This will review coding on the DoS, NHS Pathways descriptive and Service Codes proposed for change. Reviews of activity across the Region are to be analysed across the 5 ICBs to deliver an equitable service to all patients.

Other UEC Services

Provide clinical leadership at contract and performance meetings with providers when contract performance is not being met

Support the development and implementation of an integrated acute front door model that joins up services at the front door but also has strong pathways with services in primary care and community settings

Support the ongoing review Urgent Treatment Centres and the development of their role within the same day care provision

Support the development SDEC (Same Day Emergency Care) including referral pathways from primary care and the 111 and 999 services and the development of new clinical models

Facilitate the implementation of direct booking pathways from 111 and 999 to relevant services, including ED, UTC, and SDEC and UCR
