Managing and treating patients in primary and community care means fewer unnecessary admissions to hospital, so hospitals can concentrate on patients who are critically ill and those who require specialist care.

An exciting opportunity has arisen for a Primary Care Delivery Manager to join the Brent Primary Care Team in NWL to support 50 GP Practices across the borough.

The post holder will manage a portfolio of initiatives to support Practices and the wider CCG priorities.

The post holder will work with key stakeholders including; CCG colleagues, Clinical Leads, local GP Practices, Primary Care Networks (PCN), Secondary Care Clinicians and Colleagues, the Voluntary Sector and third party providers.

Main duties of the job

The successful candidate will build and manage relationships with Practices across Brent and be a point of contact to support Practices with service and general queries. They will promote innovation and champion the development of new ways of working and service delivery by supporting the Head of Primary Care, Programme Delivery Managers and the wider Primary Care team.

The post holder will be required to work flexibly to meet the needs of the team and will also be required to travel regularly across Brent for Practice visits.

Becoming an ICS will mean that strategies for Primary Care and PCN development will be developed at a North West London level but implemented locally, adapting to local needs and addressing unwarranted variation.

You will have very good communication skills, be able to produce project documentation and reports and pro-actively manage stakeholders in a challenging environment.

Job responsibilities

The primary care development and delivery portfolio will deliver an ambitious, wide ranging set of priorities that together ensure high quality, proactive and accessible primary care for the population of NW London. At the heart of the transformation programme is the delivery of resilient primary care networks (PCNs) to provide the foundation for integrated out of hospital care and population health management.

Priorities for the team will include:

· Development of effective, resilient PCNs that provide a foundation for population health management and ICP working, including embedding self-care and personalisation within local care models

· Delivery of GPFV priorities including, digital first, access and workforce

· The development of a new framework for primary care commissioning in NW London, including the ‘levelling-up’ of investment, outcome-based commissioning and a consistent offer for primary care

· Support for the GP IT programme to ensure transformation programmes are delivered in partnership with effective infrastructure and the required training and capabilities

· Delivery of national and regional targets and initiatives in line with NHSE requirements

Data-driven and outcomes-based approaches to commissioning and contracting of primary care is a core part of the portfolio and an important enabler in the transition towards a more equitable primary care landscape in NW London. The skills and expertise of the core contracting team will be key to maintaining a consistent level of business as usual alongside building a new framework for primary care commissioning.

The programme team will work in close partnership with the local primary care teams. The assistant directors of primary care in the borough teams will need to provide leadership support within the work streams and across the NW London geography as and when necessary.

Whilst primary care sits within the out of hospital programme of the ICS it extends across the majority of programme areas and will therefore need to work in a highly collaborative and flexible way supporting the delivery of the elective, urgent and emergency care, mental health, LD and autism, children and maternity and population health management programmes.

The programme portfolios reflect the agreed areas of recovery work being focused on by the NWL ICS. This role has been aligned to these areas of focus and is where post holders can expect their initial focus to be. The ICS expects to work in a dynamic and agile way and post holders will be expected to work flexible across programmes or into new programmes as these emerge. ICS priorities will be kept under review and changes to post holders work and areas of focus will be commensurate with grade.

The existing generic (Band 7) job description, person specification and knowledge & skills framework remain, however, this role specification offers information regarding current specific priorities.

• Provide a robust PMO approach to the management of programmes including:

o establishing clear project plan, timescales and outcome measures

o monitoring activity and progress against these

o providing clear reporting

o ensuring project closure is conducted thoroughly

• support the programme manager in PMO development;

All programme team members will be expected to work flexibly across the teams as and when required and the Directorate requires and actively encourages cross-working.

The role will require adopting a methodical approach to Quality Improvement through change that ensures safe, effective, patient centred timely, efficient and equitable care for patients, service users and staff.

The effects of which are measured regularly to determine whether an improvement has been achieved through system and financial benefits. Approach is to apply these methodologies to service innovation on behalf of the system to deliver strategic change to accelerate productivity by reducing waste

· Working closely with the Programme Managers to support in producing and updating programme plans, risk and issue logs, preparing risk reports, progress reports, and dependency logs.

· Attending and co-ordinating primary care meetings such as the System Oversight Meetings (SOM), GP Forum, etc. and supporting with forward agenda planning, preparing papers and presentations, and ensuring meetings, programme boards and committees operate within organisational standards

· Preparing presentations and data packs to present at PCN meetings and individual practice visits.

· Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers as required.

· Provide and receive highly complex, sensitive and contentious information, presenting information, to a wide range of stakeholders in a formal setting.

· Work with members of the team to develop and implement project data collection systems that will provide accurate and timely data.

· Working knowledge of Oracle SBS and experience of raising Purchase Orders, verifying and receipting of invoices (or a willingness to learn)
