Job summary

The Suffolk and North East Essex Integrated Care Board (ICB) is at the forefront of improving healthcare services through effective digital solutions. We are seeking a dedicated and detail-oriented Referral Pathways Facilitator to join our Digital & IT team. This role is crucial in enhancing the quality and efficiency of referral processes within primary care, ensuring the seamless delivery of healthcare services.

As a Referral Pathways Facilitator, you will be responsible for the development and maintenance of referral pathways and clinical decision support tools like DXS, Ardens and eRS. Your work will help streamline the referral process, ensuring general practice teams have access to up-to-date and accurate referral forms, which in turn supports better patient outcomes.

Main duties of the job

  • DXS and Ardens Management: Ensure the content within DXS Point-of-Care and Ardens document templates is current, clear, and aligned with the latest clinical guidance.
  • Pathway Development: Collaborate with clinical leads and transformation teams to develop and standardise referral pathways and forms across services.
  • Data Quality Improvement: Work with healthcare providers to enhance the quality of referral data, advising on the use of mandatory fields and ensuring forms are user-friendly.
  • Technical Support: Test new and updated referral forms, ensuring proper functionality and coding, and provide feedback to relevant teams. Also provide support to GP practices around the electronic referral system (eRS).
  • Training and Communication: Provide training to primary care teams on using DXS and Ardens, and regularly update them on new developments.
  • Reporting and Analysis: Monitor the usage of referral tools, analyse trends, and produce reports to drive improvements in system usage.
  • Stakeholder Engagement: Liaise with various stakeholders, including GP practices, clinical leads, and software account managers, to resolve issues and optimize system performance.
  • Clinical Systems Support: Helping the wider Clinical Systems Support team with training and advice, and system migration work on an ad-hoc basis
