Team.

Personalised Care plays an important role
in supporting people to be actively involved in managing their health and
wellbeing and to make choices that are right for them. Working across the ICB
footprint the post holder will be required to demonstrate excellent
organisational and communication skills, be flexible in their approach, able to
exercise initiative and demonstrate consistently high standards of
professionalism whilst influencing delivery of the universal model of
personalised care.
The post involves travel across the ICB to
develop relationships with key stakeholders at Alliance and Neighbourhood level,
keeping abreast of personalised care initiatives, providing support,
governance, and oversight, where appropriate.
Working as part of the ICB medical
directorate, you will be required to work remotely whilst providing project and
business support within the ICB Personalised Care Team.
Please note, due to the ongoing cost reduction programme and restructure underway at SNEE ICB (due to end in Summer 2024), prior consideration for this role may be given to existing staff who are affected by this programme and this role may be removed from the recruitment process at any time.
Main duties of the job
To actively contribute to the delivery of the ICS Personalised Care programme of work.
Support the ICS Personalised Care vision and objectives including delivery of the NHS Plan goals, planning requirements, best practice guidance and local plans and innovation.
To adopt a flexible approach working as part of the ICS Personalised Care programme to deliver projects which interface and have independencies across other Alliance programmes
To undertake any other duties or ad hoc projects of work that may occur in the team.
Facilitate and support integrated working with Alliance partners across Ipswich and East Suffolk, North East Essex and West Suffolk.
To use engagement, communication and co-production skills to ensure effective delivery with Alliance partners and people with lived experience.

Working as part of an ICS wide Personalised Care Programme across Ipswich and East Suffolk, West Suffolk and North East Essex Alliances, the role purpose is:
- Play a key role in the Medical Directorate supporting the delivery of the NHS model of personalised care.
- Support system wide work to create a shared action plan around social prescribing, creating a shared understanding of resources, evaluation and impact, delivery models and funding arrangements. Use this information to gain agreement on priority areas for future joint work, and assist with the planning and implementation of system priorities.
- Monitor and report on progress in delivery of the ICS personalised care awareness campaign and wider Alliance based personalised care training and workforce development projects
- Provide project and business support to the ICS Personalised Care Delivery programme including forward agenda planning and organising meetings and events
- Co-ordinate newsletters, briefings and updates promoting personalised care developments to system to raise awareness of progress and best practice
- Assist in co-ordinating data returns to support monitoring of programme targets and wider programme evaluation
- Establish close and constructive working relationships with ICB corporate departments and Alliance statutory providers, voluntary sector and co-production partners
